AW03003A) Dear Tim: Per our discussion at the Project Closeout Meeting on August 6, 2012, for the above referenced project, the following information will summarize the findings of a recent site inspection and the stream mitigation credits to be delivered to EEP. The following documents were reviewed to assist in preparing this information: EEP (formerly NCWRP) original RFP issued on June 25, 2002 and subsequent Addendums ■ S &N Response to the RFP (Technical Proposal, Technical Approach and Gust Proposals) submitted to EEP on October 11, ?002, and January 10, 2003 ■ Contract between EEP and S &N executed on Februan, 24, 2003 ■ Stream Mitigation Guidelines prepared by the US Army Corp of Engineers (USAGE), EEP and others and made available in April 2003 ■ Zacks Fork Stream Restoration As -Built Survey February 2006 ■ Notes from the September 28, 2011. Project Closeout Inspection with EEP. USACE. and DWQ ■ Live Stake Report and photographs prepared by ESI and submitted to EEP in November 2011 • Photographs from the March 15, 2012, site inspection ■ Caldwell County GIS data and aerial photography On August 8, 2012, Stephanie Norris of S &N and Charles Johnston of Environmental Services, Inc. (ESI) made a site inspection to document the vegetative growth of the November 2011 Live Stake Areas and evaluate potential bank failures. Live Stake Areas B — Station 38 +50 and D — Station 44+00 were found to be completely stable with established vegetation. A short section (approximately 10 LF) of bank slump was observed at Live Stake Area A - Station 34 +50. The bank at this location has already begun to stabilize and re- vegetate, as indicated by the attached photos. Therefore, we do not consider this to be a bank failure. As for Live Stake Area C — Station 41 +00, we measured approximately 70 LF of bank failure downstream of the log vane, as • �i.l. ;iili7(1 m 1';t.`: i 11i)t �j -7�?1 4 ?� lrii)ity Road ! ltaletall. �Vintll t ari'�Itrtil I t ertlh d'NonlulI J $�% pol litl.lht�• Mr rim Baumgartner Zacks Fork Stream Restoration Project August 22 2012 illustrated in the attached photos It is our opinion that the bank should be stabilized with boulders or rip -tap and the length deducted from the stream mitigation credits Please advise if this area must be repaired prior to the closeout Based on our evaluation of the above reterenced documents and the August 8 site inspection, we believe that 3,900 Lf of stream mitigation credits will be delivered to EEP This credit length is measured as follows Zacks Fork Stream Restoration ■ Beginning Station 11+35 (Downstream of Rock Vane and Bank Stabilization) ■ Ending Station 50 +00 (Bank Stabilization at the existing culvert headwall) o Total Length of Zacks Fork Stream Restoration =3 865 LF (Note Emshng u(ilit) crossings were in place and identified in the Technical Proposal and did not impede or affect the restoration of the stream At each utility crossing the stream banks and channel are stable RIP Addendum 2 dated August 30 2002, and the April 2003 Stream Mitigation Guidelines do not prohibit utility crossings of suggest the) he removed from the mitigation credit length Therefore half credit is claimed at the utility crossings The City of Lenoir Maintenance Comdor for the sewer main parallel to Zacks Fork measures approximately 12 LF to 15 LF wide This width is applied to the three, existing utility crossings at Sta 29 +25 36 +15 and 38 +90 ) Tnbutany to Zacks Fork Creek - Intermittent Stream Restoration Reference the enclosed Caldwell County GIS data and aerial photography which identifies the original alignment of the intermittent stream and the revised alignment and connection to Zacks Fork Creek The channel cross - section, pattern and profile were altered to create the new alignment This was undertaken to provide stability for the confluence of the intermittent stream with Zacks Fork Creel. The new alignment of the intermittent stream brings the alignment closer to the historic stream alignment and was included in the February 2006 As -Built Plan o Total Length of Intermittent Stream Restoration = 145 LF Stream Mitigation Credits • Total Length of Stream Restoration = 4,010 LF • Deduction — I lairCredit for 3 Emsting Utilit) Crossings = 30 LF and Pedestrian Bridge • Deduction — Half Credit for Bank Slump at Live Stake = 10 L F Area A • Deduction - Bank Failure at Live Stake Area C = 70 LF • Total Length of Stream Mitigation Credits Delivered = 3,900 LF Even with the referenced deductions the total Length of Stream Mitigation Credits Delivered to EEP meets the Contract L ength of 3 900 LF Therefore we have met the intent of die Contract and Technical Proposal Mr rim Baumgartner Zacks fork Stream Restoration Project August 22 2012 As for the Riparian Buffer and Conservation Easement the Technical Proposal referenced the existence of both a public sanitary sewer main and greenway that parallels Zacks Fort. Creel.
