Two copies of Compliance and Monitoring Form (GW-59) for the 6 wells associated with the Sludge Disposal Unit. 2. Laboratory analysis for these same wells. 3. Compliance Report Form GW-59a 4. Summary of the Groundwater Monitoring Results As shown in the summary, some of the results in the monitoring wells are above the permit limits. However, these results remain consistent with previous results and most results show decreases in concentration. The regional office has reviewed this data and requested that DAK continue sampling per the permit. No additional actions are planned at this time.
