The Division received your application for a wastewater discharge permit on August 10, 1999.
Division personrrel have reviewed and approved your application. Accordingly, we are forwarding the
attached NPDES discharge permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated December 6, 1983, and as subsequently amended.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the
legal requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

1. Upstream = at least 100 feet upstream from the outfall. Downstream = at least 300 feet downstream from
the outfall.
2. The monthly average effluent BOD5 and Total Suspended Residue concentrations shall not exceed 15% of
the respective influent value (85% removal).
3. The daily average dissolved oxygen effluent concentration shall not be less than 6.0 mg/L.
4. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units and shall be
monitored daily at the effluent by grab sample.
There shall be no discharge of floating solids or visible foam in other than trace amounts.
A. (2.) Nutrient Reduction Requirement
The Tar -Pamlico Nutrient Sensitive Waters (NSW) Implementation Strategy requires a total reduction
in nutrients (total phosphorus and total nitrogen) within the Tar -Pamlico River basin.
If requirements other than those listed in this permit are adopted as part of a future revision to the
NSW strategy, the Division reserves the right to reopen this permit and include those requirements.
If requirements other than those listed in this permit are adopted to prevent localized adverse
impacts to water quality, the Division reserves the right to reopen this permit and include those
requirements.

Describe receiving stream features and pertinent downstream uses: The creek
which cries the effluent from Macclesfield eventually mixes with the
creek which cries the effluent from Pinetops. The water from the two
creeks then travels to the Tar River, where it is upstream of Greenville's
water intake.

Spray Irrigation: Considerable amount of farm land directly adjacent to facility. Spray
irrigation could possibly be a viable alternative. The operator indicated that the Town
has been considering supplementing their NPDES permit with a Non -Discharge permit.
Connection to Regional Sewer System: The Town of Pinetops is approximately 3.5 miles
away. These two communities should be looked at for possible regionalization.

PART IV - EVALUATION AND RECOMMENDATIONS
The Raleighegional Office has reviewed the current submittal package requesting renewal
of the NPDES,permit for the Town of Macclesfield WWTP. Ourstaff has reviewed our files
of the above referenced site, performed a site visit and Compliance Evaluation Inspection, in
order to make the following comments and recommendations for the request.
1)
Please see the attached CEI letter.
2) As mentioned in the section pertaining to Alternatives Analysis Evaluation
above, there are other options available to the Town of Macclesfield. Spray
irrigation would be another option available to the Town of Macclesfield. This
option is supposedly being investigated by the Town and their engineer to use
in coordination with their existing NPDES permit. The other option is
regionalization with Pinetops. These two towns are approximately 3.5 miles
apart. It may be economically feasible, now or in the future, to combine these
two wastewater treatment systems into one larger, joint sewer authority, with
one discharge.

Based on the above, the RRO recommends that the subject project be awarded a renewal for
their NPDES permit as requested, pending NPDES Unit satisfaction of all requirements.

The subject inspection was performed on August 27, 1999, by Tim Baldwin of this office.
The cooperation of Phillip Wainwright was appreciated. Below is a numerical listing of the
findings during our inspection.
1) The current permit for the above mentioned facility became effective on
March 1, 1995, and expires February 28, 2000. The facility has a permitted flow of
0.175 MGD. The facility is a Class II facility. The ORC on record is Phillip
Wainwright.
2) The wastewater treatment process train consists of a manual barscreen, a grit
chamber, 3 influent pumps, an oxidation ditch with 2 aerating brushes, a "grease
trap", a perimeter feed clarifier, a splitter box for RAS and WAS, 2 sand filters,
chlorine gas disinfection, chlorine contact chamber, reaeration/backwash chamber,
and an effluent flowmeter.
3) The sludge handling process train consists of an aerobic digester and sludge
drying beds. Land application of solids is contracted out.
4) One years worth of DMRs (December, 1998, to November, 1999) were
reviewed for compliance with permit conditions. Violations are noted on the
attached list. Note that values reported as "greater than" are not valid results. Please
contact your commercial laboratory and explain that they need to modify their
dilutions so that these results do not occur. Please verify that the contract laboratory
has been contacted.

5) Laboratory results were checked against the DMR for one month. Everything
appeared to be correspond. The operator performs DO, temperature, chlorine, and pH on
site, the rest is contracted out to a commercial laboratory. To perform DO, temperature,
chlorine, and pH on site, your facility must have a certificate of exemption from the Lab
Certification Unit. Please verify that you have a certificate of exemption, or contact the Lab
Certification Unit to obtain one at (919) 733-3908.
6) The influent and effluent composite samplers were checked for compliance. The
influent sampler read 1 degree C, and takes time proportional samples (29 mL every 30
minutes). The effluent sampler read 1.7 degrees C, and takes flow proportional samples. It
is acceptable for the influent sampler to take time proportional samples, however, it must
take them at time intervals no greater than 20 minutes, and the sample volume must be at
least 160 mL. Please verify that the changes have been made. In addition, please describe
the flow proportional sampling method (how many mL per how many gallons flow) of the
effluent flowmeter.
7) The flowmeter had last been calibrated on March 24, 1999, which is within the one
year requirement.
8) We visited Pump Station #1 and Pump Station #2 during the inspection. Both
appeared to be operational, with on -site audible and visual alarms. The Town has a rodder
truck, and uses fire trucks to flush sewer lines. They occasionally borrow a jet -vac from
Wilson, and they have had certain problem lines smoked and televised. The Division
recommends that all lift stations have telemetry, please list which lift stations have telemetry,
and which do not.
9) During the inspection it was noticed that the digester had a considerable mat of old
sludge with vegetation growing on top. The operator also indicated that "Round -up" is
occasionally used on the sludge in the digester to reduce the vegetation. Proper operation
of all treatment units is a requirement of the permit. This office recommends that additional
mixing (capacity be added to the digester if it is unable to keep everything mixed. We also
strongly discourage the application of "Round -up" or other herbicides to any of the
biological units at the WWTP. This chemical could have detrimental effects on the operation
of the digester due to its biological nature, as well as the oxidation ditch, since digester
supernatant is pumped back through the plant. Proper mixing should prevent any vegetative
growthijfrom forming. Please provide comments on how the Town intends to address this
situation. Please be advised that federal law requires that pesticides can be used only as
directed on the label.

10) The operator indicated that he had a sufficient spare parts inventory, however, he did
not have a spare pump for the lift stations. In addition, he indicated that the WWTP did not
have a;backup generator on site. This office recommends that a spare pump be purchased
for the llift stations to expedite correcting any mechanical problems, and we also recommend
that apermanent backup generator be purchased which can run all essential WWTP
equipment in case of power loss. Please provide comments on this issue.
11) Several influent BOD values were extremely high, since we are not aware of any
industries in Macclesfield. Please explain why there are occasional high values.
Please respond in writing to items 4, 5, 6, 8, 9, 10, and 11 by March 17, 2000 detailing actions taken
by the Town of Macclesfield to address these items. You are reminded that any violation of the
NPDES permit is subject to enforcement action by this Division and/or the U.S. Environmental
Protection Ag ncy.
