Ms. Lucas,
I was attempting to change the Facility Contact person, Permit Contact person & the Permit Billing Contact person on
the above Stormwater Permit. I was trying to use the NPDES Permit Name/Ownership Change Request Form. Should I
be using a different form? It seems to want me to change the "Change in ownership of facility' or "Name change of
facility or owner".
