The North Carolina Division of Water Resources conducted an inspection of the Stateside WWTP on
3/10/2021. This inspection was conducted to verify that the facility is operating in compliance with the
conditions and limitations specified in Non -discharge Permit No. WQ0035809. The findings and
comments noted during this inspection are provided in the enclosed copy of the inspection report
entitled "Compliance Inspection Report".
Please respond in writing to this office within 30 days of your receipt of this letter regarding submittal of the
following items:
1. A Residual Management Plan that meets the requirements of 15A NCAC 02T .0707.
2. Prior to the operation of Basin #3, submittal of results of an inspection and testing performed by a
Professional Geologist.
3. ORC/BORC Designation Form for the facility.
4. GW-30 Form for the abandonment of water supply well within the compliance boundary.
5. GW-1 Forms for the installation of MW #4, #5 and #6.

Inspection Summary:
A routine compliance inspection of the Stateside WWTF was conducted on March 10, 2021. The purpose of the evaluation
was to verify that the facility is operating in compliance with the conditions and limitations specified in Permit WQ0035809.
Jeff Jarman was present during the inspection.
This 100,000 gpd Phase I Membrane Bioreactor facility start-up was in the fall of 2019. High -Rate Basins #4 ans #5 are
certified and currently in operation. Basin #3 has been constructed but not in operation, and Basins #1 and #2 have not been
constructed. A file review was completed and the following items need to be submitted to our office within 30 days:
1. A Residual Management Plan that meets the requirements of 15A NCAC 02T .0707.
2. Prior to the operation of Basin #3, submittal of results of an inspection and testing performed by a Professional

Comment: There is no caustic feed, a chlorine disinfection system, a 1,080 gallon clean in place tank or a
turbidity meter as described in the permit description. The facility uses a UV disinfection system
and there is no turbididy monitoring required in the permit.

Comment: The Influent magnetic flow meter used for reporting was calibrated in May 2020 by Delta
Systems. A new GW lowering flowmeter was installed and calibrated in September 2020 by
Delta systems.

Comment: Biological treatment is provided by a membrane bioreactor (MBR) system. Influent flows from
the EQ to an anoxic I tank to the Oxic tank, to Anoxic II tank and then to the membrane tank.
There are currently 16 membranes with 8 additional membranes on order. MLSS is usually
- 6000 mg/I. Alum and Micro C is added in the Anoxic I tank. Process control includes DO and
MLSS monitoring.

Comment: Currently our office has received the Certifications for the operation of high -rate Basins #4 and
#5. Please submit the certification for Basin #3 prior to operation. The water supply well used for
testing the basins appears to be abandoned. Please submit the required GW-30 for
abandonment of well. Monitoring wells #4, #5 and #6 are install. Please provide copies of the
GW-1 well construction forms for these wells. Freeboard in Basin #4 was at four feet and Basin
#5 was at 8 feet.
