General Permit NCG500000} is no longer required for your facility. Therefore, in accordance
with the request made by Dot Whitt of your company, CoC NCG500216 is rescinded, effective
immediately. It is the Division's understanding that your company will obtain a stormwater
permit to cover the runoff from your facility.
If in the future your company wishes to discharge wastewater to the State's surface waters, you
rust first apply for and receive a new NPDES permit. Discharge ofwastewater without a valid
NPDES permit will subject the responsible party to a civil penalty of up to $25,000 per day,

The Division of Environmental Management has recently evaluated all existing individual permits for potential
coverage under general permits currently issued by the Division. 15A N.C.A.C. 2H .0127 allows the Division to
evaluate groups of permits having similar discharge activities for coverage under general permits and issue
coverage where the Division finds control of the discharges more appropriate in this manner. The Division has
dcterrnined that the subject discharge qualifies for such coverage. Therefore, the Division is hereby issuing the
subject Certificate of Coverage under the state-NPDES general permit no. NCG500000 which shall void NPDES
Permit t'.JC0058629, This Certificate of Coverage is issued pursuant to the requirements of North Carolina and the
US Environmental Protection Agency Memorandum of Agreement dated December 6, 1983 and as subsequently
amended.
If any parts, measurement frequencies or sampling requirements contained in this general pernut are unacceptable to
you, you have the right to submit an individual permit application, associated processing fee and letter requesting
coverage under an individual permit. Unless such demand is made, this decision shall he final and binding. Please
take notice this Certificate of Coverage is not transferable. Part 11, E.4, addresses the requirements to be followed
in case of change of ownership or control of this discharge.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the
Pernuttee shall take immediate corrective action, including those as may he required by this Division, such as the
construction of additional or replacement wastewater treatment or disposal facilities. Construction of any
wastewater treatment facilities will require issuance of an Authorization to Construct from this Division.
Failure to abide by the requirements contained in this Certificate of Coverage and respective general permit may
subject the Permittee to an enforcernent action by the Division of Environniental Management in accordance with
North Carolina General Statute 143-215,6A to 143-215.6C, Please note that the general permit does require
monitoring in accordance with federal law, The monitoring data is not required 10 be submitted to the Division
unless specifically requested, however, the permittee is required to maintain all records for a period of at least
three (3) years.

The issuance of this Certificate of Coverage is an administrative action initiated by the Division of
Environmental Management and therefore, no fees are due at this time. In accordance with current rules, there
are no annual administrative and cocnpliance monitoring fees for coverage under general permits. The only fee
you will be responsible for is a renewal fee at the time of renewal. The current permit expires July 31, 1997.
This coverage will remain valid through the duration of the attached general permit. The Division will be
responsible for the reissuance of the general permit and at such time, you will be notified of the procedures to
follow to continue coverage under the reissued permit. Unless you fail to follow the procedures for continued
coverage, you will continue to be permitted to discharge in accordance with the attached general permit.
The issuance of this Certificate of Coverage does not preclude the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may he required by the Division of Environmental Management
or permits required by the Division of Land Resources, the Coastal Area Management Act or any Federal or
Local other governmental permit that may be required.

beryllim, cadmium, chromium, copper, lead, mercury, nickel, selenium,
zinc nor phEncl.
I)EARBORN 503 contains a refjned, aliphatic oil, and is the only oii
containing product in the group.
DEARCIDE 730, is a briquette microbiocide of trichl.oro-s-triacinetriane.
When dissolved in .water, chlorine is released. In your facility
DEARCIDE 730 is used in the cooiing water to maintain a 0.3--0.9 ppm
free available chlorine residual in the recirculated coaling water. Don
has been using s Hach DR100 Colometer to test the residual chlorine in
your systems monthly. To date, the highest total chlorine residual found
was 0.80 ppm.

The facility iisted above is covered under NPDES General Permit NCG500000, NCG500000 expires
on July 31, 2007. Federal (40 CFR 12241) and North Carolina (iSA NCAC 2H0105(e)) regulations require
that permit renewal applications must be flied at ieast 180 days prior to expiration of the current permit. If
you have already mailed a renewal request, you may disregard this notice.
To satisfy this requirement, the Division must receive a renewal request postmarked no later than
February 1, 2007. Failure to request renewal by this date inay result in a civil penalty assessment. Larger
penalties may be assessed depending upon the delinquency of the request. This renewal notice is being sent
well in advance of the due date so that you have adequate time to prepare your application.
If any discharge previously covered under NCG500000 will occur after July 31, 2007, the
CoC must be renewed. Discharge of wastewater without a valid permit would violate North Carolina
General Statute 143-215J; unpermitted discharges of wastewater may be assessed civil penalties of up to
$25,000 per day.

Dear Permittee:
The subject permit issued on 6/01/89 expires on 5/31/94. North
Carolina General Statute (NCGS) 143-215.1(c) requires that an
application for permit renewal be filed at least 180 days�rior to the
expiration date„, As of the date of this letter, the Division of
Environmental Management had not received an application for renewal.
If operation of a discharge or waste treatment facility is to occur
after the permit's expiration date, or if continuation of the permit is
desired, it must not be allowed to expire. A renewal request must be
submitted rio later than 180 days prior to the permit's expiration date.
Operation of the waste treatment works or continuation of a discharge
after the expiration date would constitute a violation of NCGS 143-2i5.i
and could result in assessment of civil penalties of up to $10,000 per
day. If continuation of the permit is desired, failure to request
renewal at leas18Umdays prior to expiration will result min a civil
t _
assessment of at least $250.001 larger penalties may be assessed
depending upon the delinquency of the request.
A renewal application shall consist of the following information:
1. A letter requesting the renewal.
2. The completed application form (copy attached), signed and
submitted in triplicate, referenced in Title 15 of North Carolina
Administrative Code (15A NCAC) Subchapter 211 .0105(a).
3. A processing fee (see attached schedule) in accordance with 1.5A
NCAC 211 .0105(b). The application processing fee is based on the
design or permitted flow, whichever is appropriate, listed in the
first categories of facilities.
4. Primary industries listed in Appendix A of Title 40 of the
Code of Federal Regulations, Part 122 shall also submit a
priority pollutant analysis in accordance with Part 122.21.

5. Facilities which have not been constructed within the last
permit cycle and are therefore, considered 'tnew" facilities,
shall also submit an Engineering Alternatives Analysis,
referenced in 15A NCAC Subchapter 21I .01.03 and Subchapter
2B .0201(c).
6. :f.f the facility covered by this permit contains some type of
treatment works, a narrative description of the sludge management
plan must be submitted with the application for the renewal.
In addition to penalities referenced above, a permit renewal
request received after the expiration date will be considered as
a new application and will require the higher application fee.
15A NCAC 2{{ .0105(b)(2) requires payment of an annual
Administrative and Compliance Monitoring fee for most permitted
facilities. You will be billed separately for that fee (if
applicable), after your permit is approved.
The letter requesting renewal, the completed Permit application,
and appropriate fee should be sent to:
