North Carolina Administrative Code (15.A NCAC 2H.0105(e)) requires that an application for permit renewal be filed at least 180 days prior to expiration of the current permit. To satisfy this requirement, your renewal package should have been sent to the Division postmarked no later than April 3, 2006. The Division has not received a renewal request for the subject CoC. This is a violation of your permit at Part II. B. 10, which states "Any permittee that has not requested renewal at least 180 days prior to expiration will subject the perrnittee to enforcement procedures as provided in NCGS 143-215.6 and 33 LISC 1251. et. seq.". To prevent an assessment of civil penalties you must submit a completed permit application (see enclosed forms) no later than June 19, 2006. If the system is inactive due to lack of trust fund payments, but the remediation equipment is still on site, the CoC should be renewed. PART IV- EVALUATION AND RECOMMENDATIONS On behalf of Floyd Honeycut's Estate, Handex of the Carolinas is requesting an NPDES Permit for the discharge of treated groundwater at the subject facility. NPDES Permit Staff Report Version 10/92 Page 3 It should be noted that the discharge will take place into an unnamed tributary to East Prong Rock Creek and not into an unnamed tributary to Rock Hole Creek per the application. Mr. Hertz was requested to submit a corrected map of the proposed receiving stream,. The NPEES Permit approval should be contingent upon obtaining the necessary right-of-way agreement from the NCDOT. Per our discussion, a clarification of the discharge receiving pathway is attached. Additionally, the information regarding all potable wells located within the vicinity of the former station is also included, Discharge Pathway and Receiving Waters: The treated effluent from the proposed groundwater treatment system, to be located at the former Honeycutt Exxon facility, will be discharged to a storm sewer line located within the West Main Street (NC 24/27) North Carolina Department of Transportation (NCDOT) right-of-way, From the location of the proposed effluent line tap into the storm sewer, the line travels approximately 350 feet west-southwest to a location within the First Union Bank property. At this point, the line runs south-southeast and discharges to an intermittent stream located in the southern quadrant of the First Union property, The intermittent stream is a tributary of the East Fork of Rock Hole Creek. Potable Wells Located within 1.5.00 Feet: During the preparation of Comprehensive Site Assessment, submitted to the NCDEM in March 1995, a Sensitive Receptor Survey (SRS) was performed by Handex. The purpose of the survey was to determine, to the extent possible, any potential receptors within a 1,500 foot radius of the Honeycutt site. The SRS identified 11 such receptors, both industrial and domestic. The attached figure is a summary of the original SRS findings. At the request of the Estate of Floyd E. Honeycutt, a second reconnaissance was performed in August 1996. The August 1996 reconnaissance was performed to determine the feasibility of connecting water supply well users to the existing municipal water service. The following is a summary of the second reconnaissance: 102 Locust Avenue - Site was identified in initial SRS. Potable well usage confirmed. Access to municipal water supply present. Identified as #2 on the original SRS figure. • 222 Locust Avenue - Site was identified in initial SRS. Unable to confirm uses of observed well, believed to be potable. Residence has access to municipal water, but a water meter was not observed on the property. The CAP Addendum has been signed and sealed by both a North Carolina registered Professional Engineer and Licensed Geologist. 7.e A portion of the Locust, NC USGS Topographic Map indicating the proposed discharge flow pathway is attached. The groundwater treatment system effluent will be discharged to a storm sewer located within a NCDOT right-of-way. Access from the NCDOT is pending. DRAWING 3: Piping and Instrumen Detail Sheet System Layout I:\BROWN IP\HONEYCUT\HONEYCUT.CAP ion Diagram ander 4.0 GROUNDWATER RE EDIATIQN 4.1 RECOMMENDED GROUNDWATER REMEDIATION SYSTEM To achieve hydraulic control objectives, groundwater will be pumped to create a cone of depression in the water -table. This decrease of the hydraulic head in the surfucial aquifer will cause groundwater containing dissolved hydrocarbons to migrate toward the recovery wells. Based on the capture zone analysis and well yield estimates, the proposed recovery wells should provide hydraulic control and capture of the majority of the dissolved phase hydrocarbon plume. The groundwater recovery system recommended for the site consists of pumping groundwater under vacuum -enhanced conditions from the three proposed recovery wells shown on Figure 5. As previously mentioned, vacuum -enhancement was chosen to provide capture using three recovery wells, shorten clean-up times, and remediate soils de -watered during pumping. This recovery scenario is predicted to produce an average flow of approximately 1.25 gpm per extraction well. 4.2 RECOVERY WELL DESIGN Proposed recovery wells RW-2 and RW-3 will be constructed using the same construction details as RW-1. The proposed recovery wells will be constructed of 4-inch diameter Schedule 40 PVC with flush -threaded joints. The wells will extend to approximately 55 feet below land surface (BLS), with 0.020 inch slotted well screen extending from approximately I ; \BROWN D\HOt1EYC RJT \HONEYC41T. 9 ancler 8 feet BLS to 55 feet BLS. The annular space surrounding the screened interval of the recovery well will be filled with 60/20 silica sand to approximately one foot above the screen. A bentonite seal approximately 2 feet thick will be placed above the filter pack. The remaining annular space will be filled with Type 1 Portland cement. The 8 feet of solid well casing above the well screen should provide a good surface seal for vacuum - enhanced recovery. The recovery well will be completed with subsurface piping connections. 4.3 PIPING DESIGN The pumping equipment selected will be capable of developing sufficient head to overcome the piping system elevation head and frictional losses, and will be capable of delivering more than the required flow rate of 3.75 gallons per minute (gpm) to the treatment system. All piping will be completed 24 inches below grade between the recovery wells and the treatment system compound to prevent freezing during the winter months. The groundwater discharge line from each recovery well will be conveyed to the treatment compound by one inch PVC or HDPE lines which will be manifolded at the system enclosure. A flow control valve, sampling port, and totalizing flow meter will be installed on the water line at the treatment system enclosure. Piping and valving details are shown on the recovery system detail sheet included as Drawing 2. I:\BROWN EY UT\HONEYCla-CAP 10 -Kinder' 4,4 PUMP SELECTION Clean Environment Engineers (CEE) AP-4 TL (or equivalent) controller -less, top -loading, pneumatic pumps have been selected to extract groundwater and pump it to the treatment system. These pumps are capable of producing a flow rate of approximately 1.25 gpm per well. The pump intakes will be set at approximately 30 feet below the static water level. Each pump will be equipped with a check -valve to prevent back -flow of water from the treatment system when the pump is shut down. Performance data for the specified pump is included in Appendix D. 4.5 AIR COMPRESSORS An American IMC, Model 216V, .5 horsepower, air compressor will be used to supply air to the pneumatic pumps. The air compressor will be equipped with a pressure regulator and coalescing filter, and a solenoid valve to shut off the air supply to the pumps in the event of any system failure. Performance data for the specified air compressors, air filters, and solenoid valves are presented in Appendix E. Air will be delivered to the pumps via Weatherhead Nylon 11 (or equivalent), 1/2-inch diameter nylonlpolyamid reinforced air line. 4.6 TOTAL EXPECTED WATER FLOW The normal expected flow rate for the proposed remediation system is approximately 3.75 gpm. Since the recovery pump will be set at a specific flow rate, the treatment system should not be affected by significant rainfall events and subsequent infiltration. The 1:\BROW D\HONEYCUT\IIUNEYCUT.CAI' 11 andex treatment and piping systems will be designed using a maximum flow rate of 10 gallons per minute to allow for excess system capacity. 5 0 ROUNDWATER TREATMENT SYSTEM ESIG 5.1 RECOMMENDED GROUNDWATER TREATMENT SYSTEMS The recommended treatment system consists of phase separation with an oillwater separator, a tray -type air stripping unit and a granulated activated carbon adsorber (GACA). The recommended system is shown on the P&ID presented as Drawing 1, and has been designed to meet the requirements of the General NPDES permit The treatment units required to accomplish the cleanup objectives are described in the following sections. A recovery system detail sheet is provided as Drawing 2. The relative positions of the recommended treatment equipment are shown on the system layout presented as Drawing 3. 5.2 0IUWATER SEPARATOR AND AIR STRIPPING UNIT A low -profile air stripper treatment system will provide the primary treatment for recovered groundwater using aeration. Concentrations of dissolved hydrocarbons in the influent stream were estimated from analyses of groundwater samples collected during the vacuum -enhanced pump test and the most recent groundwater sampling events. Ejector I:\BROW_D\HONEYCUT\HONEYCUT.CAP 12 cinder Systems, Inc., (ESI) was contacted to design an aeration system to achieve treatment goals based on these concentrations, at a flow rate of up to 4 gpm. Groundwater will be pumped from the recovery wells to an Ejector Systems Model SA-15 "Stripperator" (or equivalent), a modular system incorporating an oil/water separator (OWS), low -profile tray -type air stripper, controls, and enclosure into a single unit. The selected low -profile air stripper is designed for a flow rate of up to 15 GPM. The aeration unit includes an explosion -proof blower, aeration trays, a 60-gallon sump, cleanout ports, sump access port, sample port, pressure gauge, and exhaust stack. The description, schematic, and performance warranty of ESI's Model SA-15 low -profile aeration system are included in Appendix F. The specifications presented by ESI show that the selected air stripper should provide removal efficiencies above 99% for the expected influent concentrations. Treated effluent will drain to a storm sewer at the northern property boundary. 5.3 GRANULAR ACTIVATED CARBON ADSORPTION (GAGA) The secondary treatment system at the site will consist of one Hadley Aquasorb 21.8" inside diameter (or equivalent) carbon adsorbtion unit containing 200 Ibs of activated carbon. The unit is rated at 7 gpm and will be operated at approximately 4 gpm. Secondary treatment by GACA should adsorb BTEX constituents not removed by the air stripper prior to groundwater discharge to the storm sewer system. I:\BROWN ©\Ht?NEYCUT\HONEYCUT..CAP 13 andax` Concentrations of dissolved hydrocarbon constituents in the carbon influent stream were estimated using "worst case" monitoring well concentrations based on the laboratory analytical results of the most recent groundwater sampling event. Carbon consumption rates for several sizes of GALA units were then estimated using the worst case concentrations. GALA sizing calculations and carbon consumption specifications are included in Appendix G. Treated groundwater from the air stripper will be pumped to the GACA unit using a Goulds Model NPE (or equivalent) transfer pump with a 2 HP explosion -proof motor. The manufacturer's specifications for the transfer pump are included in Appendix G. The GALA unit recommended has been sized so that there is adequate capacity in the GACA vessel to treat the full estimated BTEX influent concentration (untreated) for approximately 111 days. However, the carbon bed life of this unit is expected to be significantly longer based on the anticipated air stripper removal rates. In accordance with the General NPDES Permit, the site will be visited weekly to obtain flow meter readings and to monitor system operation. The carbon treatment unit will be capable of backwashing the carbon to remove particulates and extend the carbon bed life. It is proposed to monitor the hydrocarbon concentrations in the influent and effluent to the GAC vessels, and schedule carbon replacements before hydrocarbon breakthrough occurs. Spent carbon from the GAGA unit I:\BR©NN ©\HONEYCUT\HONEYCUT..CAP 14 ndcxc will be returned to the manufa :u for regeneration or disposal. Sampling ports will be provided on the discharge line from the carbon vessel, and a totalizing flow meter will be installed on the final effluent discharge line. A particulate filter installed upstream of the GALA unit will remove solids, which will extend the carbon bed life and reduce carbon replacements. The GACA unit, in this application as a polishing unit, should receive limited hydrocarbons which should be treated by the air stripper. 5.4 PRE-TREATMENT FOR IRON AND BIOFOULING Groundwater samples collected during the pump test were analyzed for iron, manganese, total dissolved solids and total hardness to evaluate the potential for scaling and biofouling of the aeration system. The dissolved solids laboratory report is presented in Appendix H. The total manganese analysis showed a concentration of 13.2 mg/I. This concentration is considered moderate to high, and could produce a fouling problem in the air stripper. Since the residence time in the proposed OWS (106 gallon capacity) at the expected flow rate is greater than one hour, a sequestering agent will not be effective at preventing the precipitation of manganese in the air stripper. The air stripper will be inspected for evidence of manganese fouling during routine operation and maintenance visits. If manganese fouling should become a problem, the usefulness of a manganese removal system should be evaluated versus the cost of periodic stripper cleaning. TOHONEY UT.CAP 15 andcx 4 r r The groundwater iron concentration was 55.6 mgll, which is considered high, and may lead to iron fouling problems in the GALA. A quad filter unit which contains four separate bag housings all plumbed and valved together on the same chassis (or equivalent) will be installed downstream of the stripperator, before the GALA. This pre -filtering system combined with weekly scheduled filter changes and periodic GACA back flushing should solve the iron precipitation problems. The groundwater hardness concentration was 172 mg/l and total dissolved solids was 252 mgll. These concentrations are high but the quad filter should remove most of the anticipated problems. 5.5 SYSTEM ENCLOSURE, CONTROLS, AND SAFETY MECHANISMS All equipment will be housed inside a security fence for protection from vandalism. The fence shall be equipped with two 2.5 foot wide doors to allow easy access to the remedial equipment for routine inspections and cleanings. All system components in the treatment system will be explosion proof. For security purposes, the treatment system enclosure will be kept locked. A control panel will be located at the compound, and will include switches to power the air compressor, vacuum blower, and OWS transfer pump. The OWS will be equipped with high-level probes to detect if the water or oil compartments should become full. A pair of solenoid valves will be installed to automatically shut off the air supply to the pumps and I:\BROHNJAH N YCUT\HUNFYCUT.CAP 16 cindcx® dump the air pressure in the supply lines to the pumps. This condition would occur if one of the OWS high-level alarm probes is triggered, or the air stripper fails (sump full or blower fails). A treatment system layout is shown on Drawing 3. Relative locations of switches and probes are shown on Drawing 1. To regulate groundwater flow through the treatment system, a shut-off/throttling valve will be installed on each recovery well influent line and air line at the well head. An influent sampling port will be also be installed at each wellhead. A sample port will also be installed on the OWS effluent line. Piping and valving details are included on Drawing 3 5.6 STORAGE AND DISPOSAL OF RECOVERED LPH. Recovered LPH will be held in the oil/water separator for later disposal at a State approved facility. The oil/water separator will be equipped with a high level switch to shut off the recovery pump should the holding tank become full. The holding tank will be managed in such a way that LPH is periodically pumped out before the tank becomes full. All LPH handling will be conducted in accordance with applicable local, State, and Federal laws. 5.7 EFFLUENT DISPOSAL SYSTEM Treated groundwater will be discharged to the storm sewer system located along Highway 27/24 under a General NPDES permit. The CAP Addendum will be used to apply for a NPDES discharge permit to discharge treated groundwater from the site. The treatment I:\BROWJAHONEYCUT\HONEYCUTXAP 17 system effluent water -quality objectives will be the same as the criteria fisted in the NPDES discharge permit guidelines. The effluent discharge piping will be two-inch diameter Schedule-40 PVC pipe, which will discharge from the system to the storm sewer located on the northern edge of the site. Treated groundwater will flow by gravity after exiting the GACA, and no additional pumping equipment will be required. 6.0 OPERATION AND MAINTENANCE The operation and maintenance of the proposed system will consist of many tasks relating to the vapor recovery, pumping, treatment, and disposal systems and equipment at the site. A brief synopsis of the planned operation and maintenance schedule is listed in the following sections. 6.1 GAUGING AND BAILING OF WELLS Groundwater and LPH levels will be measured in all site wells quarterly and groundwater contour maps will be prepared using these data. This information will help remediation tracking and will show the development of the cones of depression around the extraction points. Those monitoring wells showing accumulations of LPH will be gauged monthly. The results of these activities will be summarized in semi-annual reports to be submitted to the NCDEHNR. I:\BR©t ^9 D\K( EYCUT\H©NEYCUT.CAP ander 6.2 RECOVERY SYSTEM MONITORING The recovery wells, piping, and treatment system will be examined during regular operation and maintenance (O&M) site visits to determine whether adjustments are needed. Site visits will be conducted on a weekly basis. Flow rates and pump intake levels will be adjusted as needed to optimize system operation. The total flow from the groundwater recovery system will also be checked and recorded. The SVE blower will be checked for operation, and the moisture separator checked and drained as needed. 6.3 TREATMENT EQUIPMENT MONITORING The treatment system is designed to function with a minimum of operation and maintenance. During O&M visits, the groundwater treatment system and vapor extraction system will be inspected for proper operation and adjusted as needed, including changing particulate filters and backflushing of the GAGA. Sampling of the system will consist of: 1) Monitoring of the hydrocarbon concentrations in the recovery system influent, midfluent, and effluent. Influent, midfluent, and effluent water samples collected monthly will be analyzed to verify treatment efficiency and permit compliance. These analyses will be conducted weekly for the first two weeks, and monthly thereafter. Influent and effluent samples will be analyzed for BTEX, MTBE, and lead. 2) Collect air samples from the soil -vapor extraction system quarterly to assess concentration changes with time. Air samples will be analyzed for BTEX, TPH, and methane. 3) Operation and maintenance of any off -gas treatment equipment as necessary. During the site checks, adjustment of the system will be performed as necessaryto maintain system efficiency and maximize operational time. I:\BROWND\HONEYCUT\HONEYCUT.CAP 1 9
