Please find attached a copy of the completed form entitled "Stormwater Compliance Inspection
Report". The report summarizes the findings of a recent inspection of the project's stormwater
controls conducted on February 18, 2008 to determine compliance with Stormwater Permit
Number SW8970303 issued on May 23, 1997.
The required actions that are necessary to bring this site into compliance are noted under the
Comments section of the report. Please inform this Office in writing within fifteen (15) days of
the specific actions that will be undertaken and the time frame that will be required to correct
the deficiencies. Failure to provide the requested information, when required, may initiate

1. Your permit expired on May 23, 2007. To date, this Office has not received a renewal request. Operation of a system without
a valid permit is a violation which may lead to an enforcement action and the assessment of civil penalties. You must request
your renewal at once.
2. The site is in need of maintenance. The slopes should have a permanent vegetative cover. Trees are allowed on the top bank
but not on the sidewall slopes. There are many trees that should be removed. Erosion is present due to the lack of a vegetative
cover.
3. There were some downspouts with drain hoses attached going down the sideslopes. This flow should be directed into the
forebay. Some of these hoses are directly across from the outlet structure which can cause short circuiting.
4. There is not a clear access to the outlet structure. It has overgrown with weeds and should be cleared. There was a great deal
of trash around the outlet structure which could cause the orifice to become plugged. Access was not possible on the inspection
date so the condition and size of the orifice could not be determined.
5. Operation and maintenance records were not available on the inspection date. You are reminded that your permit requires
you to keep records.
6. You should check the sediment levels in the forebay and remove excess if necessary.
