Comments:
Met with Mr. Godwin on the status of the renewal of the above permit. He indicated that
he had sold the property and that he no longer had anything to do with it site. I gave him
a copy of his permit and showed him where he needed to request a name/ownership
change upon selling. I also gave him all of the paperwork required for renewal. IE:
name/ownership form, renewal form, engineer's cert., and the O & M. He left saying that
he was going to discuss the site with the owner. I explained to him that if the owner
planned on doing anything different, that a modification to the existing permit would be
required.

Dear Mr, Godwin:
The Division of Water Quality issued a Coastal Stormwater Management Permit, Number SW8970116
to Godwin Concrete Co., for a High Density stormwater pond to serve Godwin Concrete Plant #4 on
2/19/1997. This permit expired on 2/19/2007, Section .1003(h) of 15 A NCAC 2H .1000 (the
stormwater rules) requires that applications for permit renewals shall be submitted 180 days prior to the
expiration of a permit and must be accompanied by a processing fee, which is currently set at $420.00.
If this is still an active project please complete and submit the enclosed renewal application in a timely
manner. If this project has not been constructed and a permit is no longer needed, please submit a
request to have the permit rescinded. If you have sold the project, or are no longer the permittee,
please provide the name, mailing address and phone number of the person or entity that is now
responsible for this permit. Enclosed is a form for change of ownership, which should be completed and
submitted if the property has changed hands.
Your permit requires that upon completion of construction and prior to operation of the permitted
treatment units a certification of completion be submitted to the Division from an appropriate designer
for the system installed. This is to certify that the permitted facility has been installed in accordance with
the permit, the approved plans, specifications and supporting documentation. Please include a copy of
the certification with your permit renewal request and processing fee. Enclosed is a copy of a sample
certification. Also enclosed is a new Operation and Maintenance agreement that should be completed
and submitted along with your renewal application.
You should be aware that failure to provide the Designer's Certification and the operation of a
stormwater treatment facility without a valid permit, are violations of NC General Statute 143-215.1 and
may result in appropriate enforcement action including the assessment of civil penalties of up to
$10,000 per day.

DETENTION POND AND COLLECTION SYSTEM
A. The detention pond will be checked after every major storm event to insure that the outlet
structure is working properly and is not clogged with debris. Remove any accumulated sediment or debris.
Any signs of erosion or deterioration of pond side slopes or any other feature must be repaired before the next
storm event.
B. The depth of the pond will be checked every six months to determine the depth of the
permanent pool. The infiltration areas will be cleaned out when silt deposits reduce the depth of the
permanent pool below 3.5 feet. The side slopes of the pond shall be mowed as required to maintain a grass
height not to exceed 8 inches in height.
C. Collection basins and piping will be checked every six months and kept clear as necessary.
3. CERTIFICATION
The owner will submit to the Department of Environment, Health and Natural Resources an Engineer's
Certification that the stormwater facilities were constructed in accordance with approved plans.

The Wilmington Regional Office received a Stormwater Manag_ment Permit Application for Godwin Concrete
Plant #4 on January 10, 1997, A preliminary review of the information submitted has found that the application is not
complete. The following information is requested in order to make the application complete:
I , Please dimension the pond's permanent pool and make sure the contour spacing reflects the proposed
dimension. My measurements iindicate that the surface area is deficient by about 1,110 square feet. (A
22' x 290' pond).
2. Please continue the 47 contour. It Jost stops on the plans. A berm must be provided for the upper portion
of the stockpile area to force the runoff into the forebay.
Please note that this request for additional information is in raTone to a preliminary review and must be received
by February 28, 1997, or the application will be returned as incomplete. The return of an application will require that all

to Godwin Concrete Co., for a High Density stormwater pond to serve Godwin Concrete Plant #4 on
2/19/1997, This permit expired on 2/19/2007. Section .1003(h) of 15 A NCAC 2H .1000 (the
stormwater rules) requires that applications for permit renewals shall be submitted 180 days prior to the
expiration of a permit and must be accompanied by a processing fee, which is currently set at $420.00.
If this is still an active project please complete and submit the enclosed renewal application in a timely
manner. If this project has not been constructed and a permit is no longer needed, please submit a
request to have the permit rescinded. If you have sold the project, or are no longer the permittee,
please provide the name, mailing address and phone number of the person or entity that is now
responsible for this permit. Enclosed is a form for change of ownership, which should be completed and
submitted if the property has changed hands.
Your permit requires that upon completion of construction and prior to operation of the permitted
treatment units a certification of completion be submitted to the Division from an appropriate designer
for the system installed. This is to certify that the permitted facility has been installed in accordance with
the permit, the approved plans, specifications and supporting documentation. Please include a copy of
the certification with your permit renewal request and processing fee. Enclosed is a copy of a sample
certification. Also enclosed is a new Operation and Maintenance agreement that should be completed
and submitted along with your renewal application.
You should be aware that failure to provide the Designer's Certification and the operation of a
stormwater treatment facility without a valid permit, are violations of NC General Statute 143-215,1 and
may result in appropriate enforcement action including the assessment of civil penalties of up to
