Please find attached a copy of the completed form entitled "Stormwater Compliance Inspection
Report". The report summarizes the findings of a recent inspection of the project's stormwater controls
conducted on March 11, 1998 to determine compliance with Stormwater Permit Number SW8 960627 issued
on August 14, 1996.
A summary of the findings and comments are noted under Section 6 of the report. Please inform this
Office in writing before April 11, 1998 of the specific actions that will be undertaken and the time frame
required to correct the deficiencies. Failure to provide the requested information, when required, may initiate

2. Inspection Date: March 11, 1998
Weather Conditions: Clear, sunny
3. Comparison of Approved plans vs. As -Built: See commnets below.
4. Maintenance Problems:
A. Additional gravel parking has been added at the front and rear of the building that has not
been permitted.
B. Detention pond banks need additional grass stabilization.
C. There is no trash rack per the approved plans.
D. Outflow orifice appeared to be clogged.
5. PE Certification Received? No Deed Restrictions Received? N/A
6. Comments:
A. Submit a permit modification for the additional gravel parking areas.
B. Stabilize pond slopes with grass.
C. Provide a trash rack per specifications.
D. Clear any obstructions from outflow pipe.
E. Submit the Engineer's Certification.

V. DEED RESTRICTIONS AND PROTECTIVE COVENANTS
Deed restrictions and protective covenants are required to be recorded for all low density projects and
all subdivisions prior to the sale of any lot. Please see Attachment A for the specific items that must
be recorded for the type of project applied for.
By your signature below, you certify that the recorded deed restrictions and protective covenants for
this project shall include all the items required by the permit, that the covenants will be binding on all
parties and persons claiming under them, that they will run with the land, that the covenant cannot be
changed or deleted without concurrence from the State, and that they will be recorded prior to the sale
of any lot.
