Please find attached a copy ofthe completed form entitled "Stormwater ComplianceInspection Report". The
report summarizes the findings of a recent inspection of the project's stormwater controls conducted on March 5,
2001 to determine compliance with Stormwater Permit Number SW8 960409 issued to you on Tune 1 I , 1996.
A summary of the findings and cornments are noted under Section 4 of the report. Please inform this Office
in writing before April 5, 2001 of the specific actions that will be undertaken and the time frame required to correct
the deficiencies. Failure to provide the requested information, when required, may initiate enforcement action. If

STORNINVATER COMPLIANCE INSPECTION REPORT
1. Project Name: I.owe's Foods (formerly Wilson's)- Oleander Drive
Project Number: SWS 960409 Control Measure: a wet detention pond
Water Body: UT Burnt Mill Creek Classification: C Sw
Directions to site; address: Corner of Oleander Drive and Fordham Road
2. Inspection Date: March 5, 2001
Weather Conditions: partly cloudy, windy and cold
3. Compliance Issues:
a. The Engineer's Certification has not been received for the pond.
b. The ownership of the property has apparently changed hands twice. The Division was not informed
of these changes, as required by the permit.
4. Required Actions:
a. Please submit the required Engineer's Certification.
b. Please complete and sign your part of the enclosed Name/Ownership change form, (i.e., Section I,
as much of Section II as you can, and sign the back as the "Current Permittee") and forward it to the
iiew owners along with the enclosed "Wet Detention Basin Supplement". The new permittee must
complete the rest of Section 11, sign on the back as the "Applicant" and sign and initial the
supplement in the appropriate spaces, and return both documents to the Division.
