On December 3, 2009, the Wilmington Regional Office received a plan revision for Stormwater
Management Permit Number SW8 080740. The revision includes the addition of a impermeable
geosynthetic liner to the constructed wetlands previously permitted for this project. it has been
determined that a formal permit modification is not required for the proposed changes. We are
forwarding you an approved copy of the revised plan sheet for your files. Please replace the old
approved plan sheet (C-506) with the new one.
Please be aware that all terms and conditions of the permit issued on August 22, 2008, remain in
full force and effect. Please also understand that the approval of this revision to the approved
plans for the subject State Stormwater Permit is done on a case -by -case basis. Any other
changes to this project must be approved through this Office prior to construction. The issuance of
this plan revision does not preclude the permittee from complying with all other applicable
statutes, rules, regulations or ordinances which may have jurisdiction over the proposed activity,
and obtaining a permit or approval prior to construction.

Dear Ms. Russell:
Greenhorne & O'Mara, Inc. (G&O) is submitting this request for a modification to existing stormwater permit
SW8 080740. This request is for review and approval of a plan revision necessary to mitigate a differing
subsurface condition discovered during construction of the above referenced project at Camp Lejeune.
A constructed wetland BMP was designed and permitted to provide stormwater management for the project.
Based on periodic on -site observations G&O has determined that the BMP has been built in accordance with
design. However, it was discovered during construction that the groundwater elevation is lower than anticipated.
As a result, the SLWT elevation is deeper than feasible to allow the constructed wetland to function as intended.
To remedy this we are proposing to have the contractor install an impermeable geosynthetic liner at a minimum
depth of 4 inches below the bottom of the deep pool and shallow water areas of the wetland to allow water to
collect and be detained to the appropriate depths outlined in the original design. The addition of the impermeable
liner is the only proposed revision to the previously permitted design.
We are requesting an express review of the included plan revision showing the location of the impermeable liner.
The following items are enclosed for your review:
• Two (2) copies of the revised sheet C-506
• One (1) copy of the narrative summarizing the revision
• One (1) check in the amount of $500.00 for the express review fee.
