Required Attachments:
1. Legible vicinity map.
2. Complete Soils Report.
3. PDF formatted site plan with the boring locations to be tested. Site plans should
be emailed or hand -delivered only. Illegible faxed maps will not be accepted.
All proposed infiltration areas and existing, active utility lines located within the proposed
basin/trench must be marked and flagged. If these areas are not flagged, the Soils Scientist
reserves the right to decline to do the investigation. If the proposed infiltration system will be
located in an area of existing pavement and there is no open area nearby, equipment capable of
breaking through the impervious layer must be provided. The soils investigation does not take
the place of a soils report prepared by an appropriate professional. The Soils Scientist will only
verify the soil conditions that are reported in the Soils Report, and make a determination as to
the suitability of the site to meet the infiltration design requirements under iVCAC 2H.1000, and
assumes no liability should the system fail.

NOTICE OF INSPECTION AND NON-COMPLIANCE

Dear Pastor Braddy:
Please find attached a copy of the completed form entitled "Stormwater Compliance
Inspection Report". The report summarizes the findings of a recent inspection of the
project's stormwater controls conducted on January 5, 2004 to determine compliance
with Stormwater Permit Number SW8 020913 issued on April 3, 2003.
A summary of the findings and comments are noted under Section 4 of the report.
Please inform this Office in writing before February 6, 2004, of the specific actions that
will be undertaken and the time frame required to correct the deficiencies. Failure to
provide the requested information, when required, may initiate enforcement action. If

system are per the approved plan. No unpermitted offsite drainage enters
the system.
❑ ❑ All the built -upon area associated with the project is graded such that the
runoff drains to the system.
❑ ❑ For ponds/infiltration, the outlet/bypass structure relative elevations are
per the approved plan. (Must be verified via as -built survey if in question).
❑ ❑ The inlets and outlet structure are located per the approved plans and do
not cause short-circuiting.
❑ X A trash rack is provided over the outlet weir and orifice.
❑ ® All slopes are grassed with permanent vegetation and are not eroding.
❑ ❑ Vegetated slopes are no steeper than 3:1.

approved plan.

❑ ❑ For low density projects, roadside swales are 3.1, vegetated and stable.
❑ ❑ For low density projects, there are no unpermitted pipes on the project.
4. Required Actions: (Explain each deficiency and what needs to be done to bring
the system back into compliance)
(a) There is no trash rack on top of the outlet structure. The trash racks are
essential in the function of the outlet system. Not only does the rack keep
trash out, it also helps keep small children and animals from failing inside the
outlet structure. A trash rack needs to be placed and fastened on top of the
outlet structure ASAP.
(b) There are several bare spots on the slopes of the pond. These need to be
grassed to prevent further erosion from taking place.
* The majority of the pond looked good, most of the sides were well grassed and
relatively erosion free. The system also seems to be working well.
