On October 15, 2010, Angela Hammers of the Wilmington Regional Office of the
Division of Water Quality (DWQ) inspected the SECU Jacksonville in Onslow County to
determine compliance with Stormwater Management Permit Number SW8 010517
issued on September 15, 2001. DWQ file review and site inspection revealed that the
site is compliant with the terms and conditions of this permit. Please find a copy of the
completed form entitled "Compliance Inspection Report" attached to this letter.
Please be advised that you are required to comply with the terms, conditions and
limitations of your Stormwater Management Permit under Title 15A North -Carolina -
Administrative Code 2H .1003 and North Carolina General Statute 143-214.7, including
operation and maintenance of your permitted stormwater system.
If the project has changed name, ownership or mailing address, a formal change of
name/ownership form must be submitted to DWQ within 30 calendar days detailing the
change. Please provide the name, mailing address and phone number of the person or
entity that is now responsible for this permit.
Please be advised that violations of your Stormwater Management Permit may be
subject to the assessment of civil penalties of up to $25,000 per day per violation. If you

I'm sorry for any conflicting information you may have received regarding the annual reporting
requirement. Please see the attached email to Pat for an explanation.
Bottom line- yes, if the Division issued the permit, then you would submit your annual reports to
the Division. There is no fee for this. If the City issued the permit, then you would send the
annual report to the City along with any fee they may charge.

Photos looking at side slopes and berm areas.
Comments:
• The photos show adequate vegetation on the pond side slopes with a
few areas of minor erosion or thinned out grass. However it appears
that these areas have been recently scarified and over seeded. PCC
recommends over seeding again in the fall any areas that still appear
to have thinned out grass.
• The grass on the berm and embankments should not cut quite so low
in the future. Ideal grass height at this pond is 4 in.
• The pond fringe, 18 in.-24 in. from the water, should only be cut twice
yearly and can be left to grow to a 36 in. maximum height.

Comments:
The pipe has been recently been repaired with new concrete & rock
under bedding. The area around the pipe was recently stabilized and
re -seeded and appeared to adequate.

• The top photo shows the recently installed 1" draw -down orifice
elbow pipe. The water level is approximately 2 in. above the normal
water drawdown orifice but the pond still draining from recent rain.
• The box was structurally sound and was free of blockages.

Comments:
• The swale has been recently dredged out and discharge from the riser
was flowing off -site. PCC recommends leaving the grass along the
swale embankments at 4" high.
