2. Inspection Date: November 21, 2003
Weather Conditions: dry and sunny
3. Compliance Issues:

❑ ❑ The drainage area (including offsite) and built -upon area to the permitted
system are per the approved plan. No unpermitted offsite drainage enters
the system.
❑ ❑ All the built -upon area associated with the project is graded such that the
runoff drains to the system.
❑ ❑ For pond slinfiltration, the outlet/bypass structure relative elevations are
per the approved plan. (Must be verified via as -built survey if in question),
❑ ❑ The inlets and outlet structure are located per the approved plans and do
not cause short-circuiting.
❑ ❑ A trash rack is provided over the outlet weir and orifice.
❑ X All slopes are grassed with permanent vegetation and are not eroding.
❑ ❑ Vegetated slopes are no steeper than 3:1.
❑ ❑ The approximate amount of permitted surface area has been provided.
(Must be verified via as -built survey or designer's certification.)

4. Required Actions: (Explain each deficiency and what needs to be done to bring
the system back into compliance)
(a) Slopes of the infiltration pond need to be grassed and stabilized to prevent
erosion.
(b) Trash has been accumulating inside the basin, this must be removed so the
Infiltration Pond can function as it was designed to do. The trash is also an eye
sore and hazard to persons in the area.
(c) Maintenance must be performed on a routine basis to the Infiltration Pond.
This is clearly stated in the Stormwater Permit that was issued to you on
September 21, 2000.

b. Trash is accumulating in the basin.
C. The slopes on the north side need to be stabilized and vegetated.
d. The City has asked you several times to fix the erosion and vegetate the basin,
beginning in September.
e. The required Designer's certification has not been submitted.

a. Remove the sediment and trash, vegetate the bare slopes, and stabilize the basin,
as required by Section H, part 3 of your permit.
