However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on March 17, 2021. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through E, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Section IIL4.a.: Please explain how the Five -Year Maximum Adjusted Plant Available Nitrogen of 30.43 pounds per dry ton was derived. B. Application Form RSC 06-16: 1. It was checked yes in Section III.3.a., please list the hazardous waste numbers. C. Land Application Site Information: 1. Pursuant to 15A NCAC 02T .1105, please provide an updated accounting of the land application sites' cumulative pollutant loading rates (CPLRs). D. Setback map: On the setback map provided, there are three residence within the Warren Tract 9-01, with 100' and 250' buffers. However, pursuant to 15A NCAC 02T .1108(c), if surface applied by vehicle, the setbacks for each habitable residence or places of assembly owned by the owner of the land is 0. Please clarify why the buffers were set. In addition, the following footnote in Attachment B does not seem to apply any longer as there is no setbacks for residence owned by the owner of the land. Please let us know if you have any objection with us removing the footnote. This property contains two residences. Residuals may be land applied to within 100 feet of the Lori Warren residence at 1407 Alexis -Lucia Road as well as the residence owned by Thomas Warren at 1401 Alexis -Lucia Road, regardless of whether a surface or a subsurface application method is used. E. Operators in Charge (ORC): 1. The designated ORC for the facility has not been on -site during Mooresville Regional Office site visits. Pursuant to 15A NCAC 08G .0204(8), please update the Operator Designation Form and follow the instructions to have a proper ORC designated.
