In accordance with your permit renewal application received December 22, 2020, I am forwarding
Permit No. WI0300162 for the continued operation of geothermal heating/cooling water return well(s)
located at the above referenced address. Please note that this renewed permit shall become effective
on April 12, 2021 until March 31, 2026, and shall be subject to the conditions and limitations stated
therein.
Please Note:
• Samples from the influent and effluent sampling ports of your geothermal well system
were collected on March 30, 2021. Analytical results will be forwarded to you once its
available.
In order to continue uninterrupted legal use of this well for the stated purpose, you should submit an
application to renew the permit 120 days prior to its expiration date. As indicated in the permit, this
permit is not transferable to any person without prior notice to, and approval by, the Director of the
Division of Water Resources. If you have any questions regarding your permit or the Underground
