This letter serves as the official response from Winston-Salem/Forsyth County Utilities to the
North Carolina Department of Environmental Quality, Division of Water Resources Notice of
Intent to Enforce Civil Penalty for the December 14, 2020 sanitary sewer overflow (SSO),
Incident #202003471, located at 3495 Braddock Court in Clemmons. The incident was a result
of pipe failure within the collection system.

On December 14 at 10:00 am, Utilities Field Operations arrived at the scene of an SSO on the
sewer outfall at 3495 Braddock Court. There was a five-foot split in the force main and first
responders requested a contractor to repair the break.
Tanglewood Lift Station was shut down to allow drainage of the force main so the pipe could be
cut and repaired. Following completion of repairs, the lift station resumed normal operations
on December 15 at 5:00am. A diffuser was deployed for 24 hours and the spill area was cleaned
following protocol.

WSFC Utilities continues to make progress in minimizing SSOs and has the proper programs in
place to continue our high level of performance. Our comprehensive sanitary sewer evaluation
study has provided strategic cleaning, and examination of our system to identify areas with the
greatest need for repair and replacement.
