4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
There is roughly 150 l.ft. of toe erosion on the left bank of Jonathan Creek adjacent to land used for recreation and a Christmas tree farm. The site is downstream of a stabilization
project completed in 2013 by NRCS on the same property. The former project has remained stable but erosion has occurred downstream since.

4h. Explain the purpose of the proposed project:*
The purpose of the proposed project is stabilize the eroding bank and establish a woody buffer. Banks will be graded to blend in to stable banks upstream and downstream of the
project length.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
A boulder toe will be installed for approximately 100 I.ft. Large boulders will be stacked from the stream bed to no higher than bank full elevation. A minimum 5 ft. wide bank full bench
well be excavated to provide flood prone area. Disturbed areas will be protected with herbaceous seeding and erosion control blanket. Woody vegetation will be established above
boulders. A track hoe with a hydraulic thumb will be used for rock placement and excavation. All work will be accomplished from the bank and no equipment should have to operate from
within the channel. Extra boulders will be placed in the channel during rock installation to direct flow away from the area of work.

Design dimensions are based on a stable section of the same stream. These dimensions have been proven to be correct based on the success of the
upstream project from 2013. Boulder toe protection has been proven in other projects to be a simple means to protect relatively short sections of
bank.

All work will be accomplished from the bank and no equipment should have to operate from within the channel. Extra boulders will be placed in the

Installation Notes: Stream Parameters:
Excavator with thumb recommended for installation.
Equipment shall work from the top of the bank.
Spoil from bank stabilization shall be hauled outside of the
floodplain and stabilized.
Acquire boulders with a flat side for stacking.
Pin erosion control matting to bank according to manufacturer's
recommendation for stream bank application with appropriate
stakes.
Acquire necessary permits prior to beginning construction.
Install woody vegetation during the appropriate season for
survivability. If installed during a period of drought, watering
may be required for vegetative success.
All plantings below bankfull shall be on 3' x 3' spacing. Use
tubes to protect from wildlife if necessary.
Operation and Maintenance:
Check the work accomplished at least once a year and after any
bankfull flow event. Contact NRCS or SWCD if any damage
has occurred to stabilization measures.
Refrain from modifying or removing any installed rock or log
strictures without NRCS or SWCD permission.
Maintain woody vegetation in established stream corridor.
Refrain from mowing or removing noninvasive vegetation.

This work consist of securing and planting permanent woody vegetation using rooted stock, unrooted stock and on site transplants.
Care and Handling
Planting stock should be stored in a cool moist environment or heeled in. Roots of bareroot stock shall be kept moist during planting
operations. Container or potted stock shall be kept moist at all times. The root balls of transplanted stock must be kept moist.
Site Preparation
Desirable trees and shrubs on the construction site should be dug and stored. On recently graded sites use temporary seedings, anchored
mulch or erosion control fabrics to provide surface protection until woody vegetation becomes established. Sod may be killed by
non -selective herbicides the year previous to planting stock (follow all federal, state and local regulations in the use of herbicides).
When hand planting; scalp an area at least 3 feet in diameter and 2-4 inches deep. Place plant in the center of the scalped area. On sites
where equipment cannot be operated safely and there are concerns about potential erosion use a non -selective herbicide to kill
vegetation in a 36 inch wide circle or 36 inch wide strip and plant in the center of the killed vegetation. If the site is a row crop site,
reasonably free of weeds, plant stock in the stubble without prior preparation. Sites with undesirable brush will need initial treatments
to facilitate planting and follow-up treatments to prevent re -encroachment.
Planting
Transplants should be replanted to the same depth they were originally growing. Plant rooted stock in a vertical position with the root
collar approximately'/2 inch below the soil surface. The planting trench or hole must be deep enough and wide enough to permit roots
to spread out and down without J-rooting. Insert dormant cuttings to the depth required to reach adequate soil moisture, leaving 2 to 3
buds above ground. After planting pack soil around each plant firmly to remove air pockets.

Streamside Herbaceous Vegetation Specifications
The work shall consist of preparing the area, furnishing and placing seed, sprigs, sod mats, mulch,
fertilizer, inoculate, soil amendments and anchoring mulch in the designated areas as specified;
streambanks, access areas and other areas disturbed by construction activities.
Seedbed Preparation
On sites where equipment can be operated safely the seedbed shall be adequately loosened and smoothed.
Disking or cultipacking or both may be necessary. On sites where equipment cannot operate safely, the
seedbed shall be prepared by hand scarifying to provide a roughened surface so that seed will stay in place.
If seeding is done immediately following construction, seedbed preparation may not be required except on
compacted, polished or freshly cut areas.
Fertilizing
Evenly distribute lime and fertilizer over the area to be seeded. Uniformly mix lime and fertilizer into the
top 3 inches of the soil. Where surface materials are predominately gravel and/or cobble, no incorporation
is required. Apply lime and fertilizer according to soil test results or at the following rates.

Sod Mats
Strip and stockpile mats of existing desirable herbaceous vegetation. Insure that several inches of sod
remains attached to the vegetation. The root systems must remain moist while the vegetation is stockpiled.

Temporary Seeding - Use where needed for erosion and pollution control, when permanent
vegetation cannot be established due to planting season and where temporary ground cover is needed to
allow native or woody vegetation to become established.

treated with a cold wet chill process to maximize germination. Fall plantings should be planted with a
lightly sowed cool season annual to hold the soil and do not need the wet chill process. To improve wildlife
habitat existing stands of fescue can be killed using an appropriate herbicide and warm season grasses can
be no -till planted into the sod. Omit fertilizer when establishing native grasses, fertilizer can be applied the
second year to improve the stand.

Cultural resources survey for planned channel bank stabilization, streambank protection, and critical area
treatment.
List of Findings: V.
A preliminary literature search revealed potential NRHP many sites recorded in close vicinity and a high potential land form for CR's.
A cultural resources survey of the planned APE included visual investigations of the river bank along the planned APE and shovel
testing. Both visual and subsurface investigations revealed that a significant amount of fill had been placed along the APE. The fill
sets atop a very gravely creek bed. The high gravel content of the soil apparently extends a good distance to the west from the APE.
The adjacent area has been mined for this gravel. No evidence of CR's was discovered.
Recommendations:
Continue with planned assistance. Should any prehistoric or historic cultural resource(s) or human remains be uncovered during the
ground disturbing activities inherent in this project, all work must cease in the area of the discovery. Steps should then be taken to
secure and preserve the discovery and the Cultural Resources Specialist should be contacted ASAP.

On Jonathan Creek along HWY 276, about 150 feed of the left bank
has toe erosion and near vertical banks. Large toe boulders will be
installed to protect the toe of the bank. Bank sloping and a bank full
bench with woody vegetation established should proved long term
stability.
