Were samples collected from Influent/Effluent sampling ports? ® Yes ❑ No.
Provide well construction information from well tag: Well construction information on file, indicates
the well was installed 8/18/2010 to a total depth on 645 ft. BLS. The well is 6" diameter has 110 ft. of
casing. The well yield is 8GPM and static water level is 60 ft. BLS. The tag was not readable on the site
visit on 3-30-21.
b. Does existing or proposed system use same well for water source and injection? ® Yes ❑ No

IV ADDITIONAL REGIONAL STAFF REVIEW COMMENTS/ATTACHMENTS (Optional /If Needed)
Water samples were collected from the Influent and Effluent ports of the geothermal well. The water samples
were submitted to the DWQ Water Quality lab for analysis. Samples will be analyzed for a variety of
parameters. The laboratory was notified of parameters with short hold times such as Total and Fecal Coliform
bacteria and Total Nitrate as (NO3 + No2-N).
Laboratory reports will follow once all of the analysis have been completed. If elevated constituents are
discovered by the sample analysis, the permittee will be notified regarding corrective actions that may need to
be taken to correct elevated constituent reported from the lab analysis.
The MRO recommends the renewal of this permit.
