CF - Letter dated 5/4/94from Lyn Hardison to Russ Chappell told him that he was non-
compliant for not monitoring for everything on his permit.
Compliance data: No violations, but all values for the following have been the same every
month for the last two years: Q,= 0.047, chloride = 72.0, and iron = 120.0
Chloride is listed as 72.0 mg/1 on compliance sheet and as 72.0 ppm on DMR.
• Facility is monitoring for TSS monthy instead of bimonthly. *
• Facility is not monitoring for settleable solids. *
• No Turbidity monitoring; weekly U&D required. ORC will start this.
• Testing for Chlorine although it is not required; all data = 0.
• DMR's submitted only for 001. Which is this the softener or the sand filter
discharge? Is any monitoring performed at the other discharge?
Two plants WTP #1 and WTP #2, one outfall at each plant
• The data for chloride (72 ppm) is where the operator is writing Chlorine on the
DMR's. He has a column for chloride and one for C12 Difference is?
Chlorine should be written as chloride
• Is alum flocculation used? No

Barry Adams and Kimberly Hall visited @ 3 wks ago and talked to Richard Copeland
about these items. ORC will start monitoring for turbidity, settleable solids and TSS
bimonthly. He stated that he will start using exact #'s on the DMR instead of rounding to
the same ones every time.
CF Notes:
Last SR 8/4/92 Lyn Hardison Weekly eff monitoring was requested for chloride and
salinity, and U & D monitoring.
• U & D was put in for Turbidity only.
• Salinity monitoring was not put in the permit.

11/21 Kim Hall called. Facility does have 2 outfalls. 001 is the permitted sand filter discharge.
002 will be added to the permit as the water softener backwash discharge. Brine recharge so

Dear Mr. Chappell:
In accordance your request for an NPDES permit renewal received on July 20, 1997, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated December 6, 1983.
The fallowing changes have been made to the above referenced permit:
• Weekly Total Residual Chloine monitoring was added.
• The pH monitoring requirement was removed.
• Upstream and Downstream monitoring locations were added for instream monitoring.
• A second outfall was identified during a site visit by Ms. Kimberly Hall of the Washington
Regional Office. An effluent Iimitations page for water softener backwash, outfall 002, was
subsequently added.
• The chloride monitoring requirement was removed from outfall 001.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B'of the North Carolina General Statutes, and filed with the Office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this
decision shall Abe final and binding.
Pleasetakenotice that this permit is not transferable except after notice to the Division of Water
Quality. Part II, E.4. addresses the requirements to be followed in case of change in ownership or control
of this discharge. The Division of Water Quality may require modification or revocation and reissuance of
the permit.
This permit does not affect the legal requirements to obtain other permits which may be required by
the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area
Management Act or any other Federal or Local governmental permit that may be required. .

Parts I, II, III, and IV hereof.

1. Continue to operate sandfilters used to remove solids, discharge sandfilter backwash from
outfall 001 and discharge softener backwash from outfall 002 located at Perquimans County
Bethel Water Treatment Plant #1, on NCSR 1340, south of Bethel, Perquimans County (See
Part III of this Permit), and
2. Discharge from said treatment works at the location specified on the attached map into Bethel
Creek which is classified C-Swamp waters in the Pasquotank River Basin.

PART IV — EVALUATION AND RECOMMENDATIONS
It is noted that the existing water treatment plant and wastewater facility
will be replaced in the future although the specific time is unknown.
The permit states that there is one discharge site. The Bethel plant
presently has two discharge sites. One pipe discharges water from the
softener system, while a second pipe discharges from the sand filters.
It is recommended to re -issue NPDES permit NC0068861 to Perquimans County
Water Department with a second discharge.
