Permanent ground cover sufficient to restrain erosion has been established. No additional self -monitoring or self -
inspections are required. Project is in compliance and released under the SPCA. Termination of coverage under the NPDES
General Stormwater Permit for Construction Activities (NCGO1) must be requested by the permittee via a Notice of
Termination (NOT). The NOT is available at: hops://edocs.deg.nc.gov/Forms/NCGOI-Termination. Please note that this
correspondence must be uploaded as part of completing the NOT process.

City of Monroe Staff has inspected the above -referenced site for erosion and sediment control measures. The above -referenced item(s) of non-compliance
must be addressed immediately. If these issues have not been addressed within N/A calendar days of this notice, further action will be taken.
You are ultimately responsible for preventing off -site sedimentation. If off -site sedimentation occurs, even within this grace period, the City of
Monroe may assess a civil penalty. The Monroe Code of Ordinance §158.18 allows penalties of up to $5,000 per day of violation to be assessed.
Penalties continue to accrue until the applicable violations are corrected.
NOTICE:
For sites one acre or more in disturbance. Erosion and sedimentation control facilities shall be inspected by the responsible party at least once
every seven (7) days or within 24 hours of a storm event greater than 0.5 inches of rain per 24 hour period. Please reference NPDES General
Permit NCGO10000 and Section 158.17 (F) of the City of Monroe Erosion and Sedimentation Control Ordinance. Failure to maintain erosion
control devices can and will result in assessment of civil penalties.
