0.80 MGD dual contact stabilization wastewater treatment
facility with a 0.5 MGD concrete plant & 0.3 MGD steel plant (73.8% domestic &
26.2% industrial) consisting of: a combined headworks system (max 2.0 MGD)
with a fine screen assembly,.aerated grit chamber, mechanical barscreen; by-
pass manual bar screen, & flow splitter box; contact & reaeration basins with
two 105 HP blowers & one 125 HP blower providing course bubble diffused air;
clarifiers with sludge returns; chlorine gas (150 lb cylinder) chemical feed
equipment utilizing potable water for injection & an automated chlorine residual
analyzer ; 10,900 gal chlorine contact chamber (for the 0.5 MGD plant) and a 6480
gal chlorine contact chamber (for the 0.3 MGD plant) and a 280 gal
dechlorination tank with gaseous chemical feed equipment for diffused sulfur
dioxide injection; (UV disinfection equipment — decommissioned); 400 KW
standby generator with automatic transfer switch; ...

deficiencies: The plant had several months of of non-compliance for flow.
B. By month, list of the number and type of any violations of permit conditions,
environmental regulations or environmental laws, including (but not limited

to):
1. Permit Limit Violations (NOVs and Civil Assessments):
Feb, flow, one weekly and one monthly non- compliance for BOD.
April, May, July, August, and September were non-compliant for flow.
October was non-compliant for BOD.
2. Monitoring and Reporting Violations (NOVs and Civil Assessments)

III. Notification:
This annual report has been sent to the North Carolina Division of Water Quality and to the owner of
the wastewater system. The wastewater system owner has also been given a letter that they can easily
distribute to all their customers. The letter will inform the customers that the annual report is available
upon request to the owner of the system

The SYSTEM PERFORMANCE ANNUAL REPORT(SPAR) is completed! The SPAR is a report that
lets us, the wastewater treatment provider, tell you, the user, what has been happening with our wastewater
plant in the past year(2020). In the SPAR,we give you a brief description of the treatment plant, if we had any
violations, and what was done about any violations. If you would like to have a copy of the AWTR,please call
