MECKLENBURG COUNTY Department of Environmental Health December 11. 1986 Mr. Dick Hinson Sync© Properties Post Office Box 34487 Charlotte, North Carolina 28234 Dear Mr. Hinson: On December 1, 1986, our department made an uation of the wastewater treatment facility at Carmel Center. There was no discharge from the facility to the ca brination chamber. There was a discharge to McMullen Creek, indicating an inflow of ground water to the discharge pipe. The following items need to be addressed: 1. Plants and silt were present on the sand filter beds. As noted in our letters beginning in January 1984, this has been a continuing problem that has not been addressed. 2. Drain tile was displaced. This tile was placed back in place by Decemb 5, 1986, however the joints were not partially seal:' All of the joints need to be checked under flow co `+ '`'?: ons to insure proper dosing across the sand beds. Plugged drain tiles were still present. All tiles need to b,..leaned and flushed to insure proper dosing. 4. Distribution box needs a lid and should be checked for water tightness. 5. The septic tank and dosing chambers need to be checked, the septic tank pumped, and the dosing siphons checked for proper working condition. Depending on the amount of time that has gone by, the dosing chambers may also need to be pumped out and cleaned. 6. Mecklenburg County regulations require wastewater treatment facilities to be fenced so as to prevent access by people and animals. The vandalism of the plant noted on December 1, 1986 needs to be prevented in the future. There is also trash present near the manholes to the septic tank and 1200 Blythe Boulevard Charlotte, North Carolina 28203 • (704) ,376-4603 dosing chambers raising the possibility of trash being thrown into the system. 7. The septic tank manhole has a hole at the side of the manhole which needs to be fixed. We request that you notify this Department concerning the operator who is in charge of this plant. We would also like to work with you and the operator in resolving the above deficiencies noted at the plant. Please contact myself or Mr. Rusty Rozzelle at 376-4603. Hilton, Regional Manager DIVISION OF ENVIRONMENTAL MANAGEMENT September 24, 1986 Subject: Failure to Submit Monitoring Reports Carmel Center Mecklenburg County, NC A review of this Office's files shows that NPDES Permit No. NC 0050857 was issued on July 19, 1982 for the discharge of wastewater from the subject facility. The NPDES Permit requires monitoring of the facility and the submission of monthly monitoring reports on forms furnished by this Agency. Our review indicated that monitoring reports are not being received on a monthly basis. If you no longer have a discharge then you should request that the Permit be rescinded, otherwise, reports must be submitted monthly. If you do not have a discharge for a particular month, then indicate "NO DISCHARGE" on the report. Failure to submit monitoring reports for the parameters, frequencies and locations set forth in your NPDES Permit is a violation of the Permit and is subject to civil penalties of up to $10,000 per day per violation. In light of your noncompliance status, it is requested that a written response be submitted within 15 days of the date of this letter. Your response should explain reasons for noncompliance and action being taken to achieve compliance. Failure to provide an adequate response will leave this Office no alternative except to recommend enforcement action. Please direct your response to the attention of Mr. Thurman Horne. 919 North Main Street, P©.
