Enclosed is a copy of the Compliance Evaluation Inspection report for the inspection conducted
at the subject facility on March 23, by Ori Tuvia. Your cooperation during the site visit was much
appreciated.
The following deficiencies were found during the inspection and should be corrected:
1) Change of ownership of the permit is needed, form was given at the time of the inspection.
2) The pipe connecting the sand filter to the chlorine tablets appeared to be broken and need
to be repaired.
3) Wastewater chlorination tablets are needed, example of approved tablets is attached.
4) The effluent pipe appeared to be clogged with mud and need to be cleared.
5) Annual sampling of the discharge is needed approved list of labs is attached
6) Records of sampling (flow permitting) and septic tank being pumped should be kept so it
can to be reviewed in future inspections.
