The City of Statesville is proposing to repair and extend an existing
culvert that runs under Columbia Street in the City of Statesville.
Statesville is proposing to flatten the near vertical embankment
slopes to an approximately 3:1 slope immediately north and south of
Columbia Street, extend the existing culvert by approximately 28 LF
downstream and by 20 linear feet upstream, and install inlet
protection against ongoing scour upstream. Repairs on the
downstream end of the culvert would include installing a drop
manhole structure at the existing culvert and extending the culvert
downstream at grade. Alluvial soils related to ongoing erosion would
be removed from the construction area as necessary and the culvert
end -wall would be repaired or replaced. The preliminary design set
has been attached to this Pre -Filing Meeting Request.
The 60-inch culvert proposed for repair and extension conveys
surface water within an unnamed tributary that runs under Columbia
Street to Third Creek located downstream to the south. The City of
Statesville anticipates on submitting a pre -construction notification to
the U.S. Army Corps District Engineer and NC DEQ Division of Water
Resources prior to commencing with culvert repair.

GENERAL NOTES:
1.ALL WORK SHALL BE PERFORMED IN ACCORDANCE WITH THE MOST RECENT VERSION OF NCDOT 2018 SPECIFICATIONS AND SPECIAL PROVISIONS, AND IN ACCORDANCE WITH APPROVED

14.CONTRACTOR SHALL ENSURE THAT ALL WORK SHALL BE PERFORMED ACCORDING TO CURRENT OSHA SAFETY REGULATIONS. CONTRACTOR SHALL PROVIDE SAFETY TRAINING CERTIFICATION
DOCUMENTATION IF REQUESTED BY THE CITY.
15.CONTRACTOR SHALL COORDINATE WITH THE ENGINEER, THE CITY, AND ADJACENT PROPERTY OWNERS AS REQUIRED FOR WORK ON PRIVATE PROPERTY.
16.CONTRACTOR SHALL BE RESPONSIBLE FOR LAYOUT OF ALL WORK AS SHOWN ON PLANS AND SHALL VERIFY ALL DIMENSIONS AND GRADES AT THE JOB SITE. IF DIFFERENCES ARE FOUND, THE
CONTRACTOR SHALL NOTIFY THE ENGINEER IMMEDIATELY SO THAT MODIFICATIONS CAN BE MADE.
17.CONTRACTOR SHALL MAINTAIN A SET OF ANNOTATED DRAWINGS SHOWING ANY FIELD CHANGES TO THE PROJECT. THE CONTRACTOR IS NOT AUTHORIZED TO MAKE FIELD CHANGES

18.UTILITIES WERE LOCATED BASED ON FIELD MARKINGS PROVIDED BY 811 ONE-CALL. NO SUB-SURFACE LOCATIONS HAVE BEEN PERFORMED. ALL SUB-SURFACE UTILITY LOCATIONS ARE
APPROXIMATE.
19.CONTRACTOR SHALL BE RESPONSIBLE FOR LOCATING ALL EXISTING UTILITIES PRIOR TO CONSTRUCTION AND NOTIFYING APPROPRIATE UTILITY PROVIDER OF ANY CONFLICTS. ADDITIONALLY,
THE CONTRACTOR SHALL BE RESPONSIBLE FOR EXERCISING REASONABLE EFFORTS TO PROTECT ANY KNOWN AND UNKNOWN UNDERGROUND FEATURES. THE CONTRACTOR SHALL NOTIFY

21.CONTRACTOR SHALL BE RESPONSIBLE FOR PROTECTION OF THE PROJECT FROM EFFECTS OF FLOOD AND STORM EVENTS, INCLUDING BUT NOT LIMITED TO WORK IN PROGRESS, COMPLETED
WORK, EQUIPMENT, MATERIALS, UTILITIES, WITHIN THE PROJECT AREA OR AFFECTED BY THE PROJECT OR DAMAGED AS A RESULT OF THE PROJECT.
22.TRAFFIC CONTROL DEVICES, SIGNS, LANE CLOSURES, SIDEWALKS, ETC. SHALL BE INSTALLED IN ACCORDANCE WITH NCDOT STANDARDS.
23.THE CONTRACTOR SHALL BE REQUIRED TO FURNISH, INSTALL, RELOCATE, AND MAINTAIN ALL TRAFFIC CONTROL MEASURES (SIGNS, BARRICADES, ETC.) FOR WORK SITES AS REQUIRED. THE
LOCATION AND POSITIONING OF THESE TRAFFIC CONTROL MEASURES SHALL BE IN ACCORDANCE WITH THE PLANS AND APPROVED IN FIELD BY THE ENGINEER BEFORE CONSTRUCTION BEGINS.
24.THE CONTRACTOR IS RESPONSIBLE FOR FINAL CLEANUP WHICH SHALL CONSIST OF REMOVING ALL TRASH AND MATERIALS INCIDENTAL TO THE PROJECT AND MOVING THEM OFFSITE.
25.THE TEMPORARY EASEMENT DOCUMENTS SHOWING BOUNDARY BEARINGS AND DISTANCES WILL BE PROVIDED TO THE CONTRACTOR.

2.SET UP TRAFFIC CONTROL FOR DOWNSTREAM CONSTRUCTION, ADDING GRAVEL AS NEEDED TO BUILD UP AND
PROTECT UPSTREAM SHOULDER.
3.INSTALL JUNCTION BOX, DOWNSTREAM PORTION OF PIPE CULVERT EXTENSION, AND CONSTRUCT ENDWALL.
