This Application cannot be used for wetland or stream impacts. Please provide seven (7) copies of this application and supporting information as well as a non - refundable application fee to the Division of Water Quality of $240.00 for impacts of to lake bottom of less than 1 acre and $570.00 for impacts to lake bottom of greater or equal to 1 acre. The current foundation wall has failed leaving the concrete pad unsupported. The wall is located lakeside and behind the boathouse wall and will require a pour into the lake area inside the boathouse.. The proposed repair plan was engineered by Kim Warner and a sealed plan is attached for your review. I have included photos of the site see page a. To facilitate the pouring of a new wall via engineered plan a series of sand bags will be placed in the lake in the work area and all lake water will pumped out so no concrete can enter the lake. The pump will be on site at all times and no debris will be allowed to remain in the lake after the wall is poured. The site of the pour is fairly shallow 2 -21/2 feet and there are stairs directly to the site so no vegetation or trees will be affected in any way. TO STABILIZE THE STRUCTURE, A NEW REINFORCED WALL WITH A PIER - SUPPORTED FOUNDATION WILL BE INSTALLED AND SECURED TO THE FAILING FOUNDATION WALL, THE PURPOSE OF THIS REINFORCED WALL WILL BE TO TWOFOLD: IT WILL SERVE TO TIE THE FAILING FOUNDATION WALL PARTS TOGETHER AND ALSO TO PROVIDE SUPPORT POINTS FOR THE (4) NEW 50K HELICAL PIERS THAT WILL ALSO BE INSTALLED INTO THE LAKE BOTTOM. THE FOLLOWING IS AN OUTLINE OF THE STEPS REQUIRED TO COMPLETE THIS PROJECT. THIS OUTLINE IS NOT MEANT TO BE A STEP BY STEP WORK PLAN BUT A DOCUMENT THAT HIGHLIGHTS THE ITEMS THAT THIS ENGINEER HAS CONCLUDED WILL BE IMPORTANT TO ENSURE SUCCESSFUL COMPLETION OF THE PROJECT. 1. PROVIDE SUFFICIENT SUPPORT FOR EXISTING ROOF AND DECK SYSTEM TO FACILITATE THE REMOVAL OF WOOD WALL AND SUPPORT PIERS FROM NORTH END OF BOATHOUSE TO SOUTH END (APPROXIMATELY 24') 2. REMOVE ANY LOOSE DEBRIS THAT MAY FALL OUT OF THE EXISTING WALL DURING CONSTRUCTION 3. INSTALL 50K HELICAL PIERS AS PER LAYOUT AS CLOSE TO EXISTING WALL AS POSSIBLE 4. SURVEY THE FOUNDATION WALL FACE AND DETERMINE HOW FAR AWAY FROM THE FACE THE CONCRETE FORM SHOULD BE SECURED FROM THE WALL. THE FORM SHOULD BE PLUMB AND SHOULD BE LOCATED TO PROVIDE THE LEAST TOTAL OVERALL THICKNESS BUT PROVIDE A MINIMUM OF 8" OF COVERAGE OVER THE ENTIRE AREA 5. INSTALL #5 REBAR PIN ANCHORS WITH HILTI BRAND HY -150 EPDXY TO A MINIMUM DEPTH OF 4" INTO EXISTING FOUNDATION ON 18X18 INCH CENTERS AS SHOWN. FROM SURVEY IN #4 ABOVE DETERMINE THE PIN LENGTH REQUIRED SUCH THAT PIN WILL BE TWO TO THREE INCHES FROM INSIDE FACE OF FORM WHEN INSTALLED. 6. #4 REBAR SPACED ON 12" CENTERS SHOULD BE INSTALLED HORIZONTALLY AND #4 REBAR SPACED ON 18" CENTERS SHOULD BE INSTALLED VERTICALLY. THESE REINFORCING BARS SHOULD BE INSTALLED IN THE APPROXIMATE CENTER OF THE NEW CONCRETE 7. THE ONE SIDED CONCRETE FORM SHOULD BE SECURED TO THE FOUNDATION WALL USING APPROPRIATE ANCHORS (WILLIAMS FORM ENGINEERING HAS SEVERAL GOOD CHOICES). THE ANCHORS USED MUST HAVE SUFFICIENT LATERAL STRENGTH, IN TOTAL, TO SUPPORT THE WEIGHT OF THE CONCRETE IN THE FORM AND BE REMOVABLE SUCH THAT THE REMAINING HOLE OR REMNENT OF ANCHOR PART CAN BE COVERED AND HIDDEN WITH GROUT ONCE THE FORM HAS BEEN REMOVED.
