NCO059251 I11 12I 21/03/30 117 18 i c i Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) On March 30th, 2021, Alex Lowe of the NC Division of Water Resources conducted an inspection of the wastewater treatment plant at Quail Acres Mobile Home Park (NPDES permit no. NC0059251). ORC Anthony Montero was present. The plant appeared to be well operated and maintained. Lowe observed the influent pump well to be free of scum and grease buildup, and Montero manually raised the high-level float to demonstrate that the high-water alarm was functioning. The sand filter bed was free of ponding or tunneling, and Monten noted that he manually alternates the arms of the trickling system to which he feeds, allowing for better control over wastewater distribution. Montero stated that he has plans to remove, clean, and replace the sand every year. Lowe observed that a segment of fencing on the west side of the plant, just beyond the sand filter, was cut open and pulled apart, to allow free passage through the back of the plant grounds. Lowe noted to Montero that the fence must be mended, and a lockable gate must be installed if the operators wish to have egress on that side of the site. Montero agreed, and stated that he would have is BORC close the hole as soon as possible. Both the chlorine contact chamber and the dichlorination chambers were clear of buildup. Lowe confirmed that the tablets in use were the proper wastewater -grade tablets. Effluent was free of visible solids, and receiving waters showed only trace amounts of bubbles directly under the outfall. No foam or bubble were observed more than a few feet from the outfall. A review of the provided data files suggests good documentation practices. Lowe compared third -party laboratory results for 10/2020, 11/2020, and 12/2020 to DMR data, and found no sign of misreporting o DMRs. While debriefing, Mr. Montero acknowledged the struggles the plant faced with maintaining compliance in 2020 and explained that his contract with the plant only began in January of 2021. In the first quarter of the year, Mr. Montero has orchestrated the replacement of the sand filter bed, the calibration of the flow meter for the first time in several years, and balancing the chlorination/dichlorination of effluent. Mr Montero stated that the owner, Mr. Lee Simaan, is willing to invest in maintenance and improvements on the plant and indicated that previous violations were due to lack of communication between the operator and Mr. Simaan.
