Snider, Regional Supervisor North Carolina Department of Environmental Quality Division of Water Resources 450 West Hanes Mill Road, Suite 300 Winston-Salem, North Carolina 27105 Re: Sanitary Sewer Overflow Civil Penalties Incident #202002757 Dear Mr. Snider: ., anent of'.;" ;!ityi This letter serves as the official response from Winston-Salem/Forsyth County Utilities to the North Carolina Department of Environmental Quality, Division of Water Resources Notice of Intent to Enforce Civil Penalty for the October 8, 2020 sanitary sewer overflow (SSO), Incident #202002757, located at 644 Arbor Drive in Winston-Salem. The incident was a result of pipe failure within the collection system. On October 8 at 11:07am, Utilities Field Operations was notified of an SSO at 644 Arbor Drive. There was evidence of sewer and stagnant water in the storm drain. Dye tests were conducted on the sewer lines, but no dye appeared in the storm drain. First responders were unable to visually locate the leak. A CCTV crew responded and quickly inspected the sewer main to find the cause of the pipe failure. During their investigation, a defect in the pipe was found that allowed wastewater to leak out of the pipe and into the nearby stormwater system once the level rose above three inches. To resolve this issue, permanent repairs were made to the pipe and all leaks were fixed. A six- foot section of six-inch pipe was also replaced. A diffuser with dechlorination tablets was deployed for more than 12 hours and the spill area was cleaned following protocol. Our comprehensive sanitary sewer evaluation study has provided strategic cleaning and examination of our system to identify areas with the greatest need for repair and replacement.
