Dee Freeman Secretary The Division was notified on September 16, 2011 that the subject facilities were no longer :receiving influent :flows after July 25, 2011 (McCarron WWIP) and August 25, 2011 (Willows Creek WWTP). Site inspections were performed at both facilities on October 5, 2011 by Mr. Wes Bell of this Office to determine the status of each facility. Also in attendance during the site visits were Messrs. George Brant (Senior Project Manager) arid Travis Gass (inspector), Charlotte Mecklenburg Utilities (CM(..1), and Messrs. Andrew Martin and Jeff Mitchell, Mecklenburg County Water Quality Program. Both treatment facilities were shut down with all wastewater being diverted into the CMU collection system. 'The only wastewater remaining at either facility was observed in the former influent pump station at the McCarron WWTP. The treatment tanks/units (both facilities) had been previously cleaned by the contracted operations of Bionomics that included the initial power washing of each tank along with a final rinse with a diluted bleach solution. The treatment tanks appeared to be adequately cleaned and no evidence of previous wastewater discharges/spills was observedon the site grounds. H.Sorensen took the caller's number, noother information was given. H. Sorensen attempted tocall the reporting party and discovered the number was wrong. H. Sorensen observed one wastewater treatment plant between 185 ard US74 along 1435, off of5LanwoodAvenue, When H.Sorensen arrived onscene, large puddles ofwater were observed. |tappeared that the discharge had stopped, Wet tissue was observed around the edge ofone ofthe treatment tanks. Water was dripping fromthis area. See Figures 1,2,3,and 4.|tappeared that alarge discharge had occurred and facility personnel had responded and corrected the discharge. Mosolids were observed onthe ground, See Figures and 6. The discharge may have reached the Reedy Creek Tributary. H. Sorensen inspected the pump station |pcnted at I0725Star*xxad Avenue, This area appeared to be ingood condition. See Figures 7and B. Signs were posted with Heater Utilities Inc. information, Two toll free numbers were posted. Both were out ofservice. Attempts were mode tocontact the company; however, numbers found online were not useful. The property is located at the end of Starwood Avenue and is owned by the Department of Transportation and Heater Utilities, Qostreet number |sgiven for the address, 11`8-20I0� Ron Eubanks and Drew Martin (MCWQP) drove to the Willows Creek Subdivision WWTP to perform site inspection, Upon arrival at the site, Ron Eubanks and Drew Martin met with Mr. Kevin Curl (ORC-AquaNorth Carolina, |nc). Mr. Curl indicated that excess solids and debris clogging the inlet bar screen (Photo 2)caused the discharge from the Willows Creek Subdivision VVVVTP(Photo Z). During the site inspection, excess algae and/or flocculent was observed in the clarifier units (Photo 3)and UV disinfection chamber (Photo 4). After inspecting the WWTP, Ron Eubanks and Drew Martin inspected the creek, From avisual inspection, the discharge from the VVVV7Pdid not appear to impact the creek; however, there was some staining onthe rocks and streamchannel below the VVVVTPmotfaU(Photos 5 &6i After inspecting the VVVVTPand creek, Ron Eubanks and Drew Martin explained to Mr. Kevin Curl that the sewage oil the ground would have to be cleaned up and disinfected with hirne; it was also recommended that the solids observed inthe creek below the VVWTPoutfuUberemoved, 1,1-9-7010: Ron Eubanks drove back the Willows Creek Subdivision WWTP to perform follow-upsite inspection. From the site inspection, the sewage that had discharged onto the ground had been cleaned upand the affected area was covered with lime (Photos 78^8), After inspecting the clean uparea, Ron Eubanks inspected the creek below the VVVVTPuutfaU. From avisual inspection, the creek appeared to be slightly turbid; however, the turbidity was observed upstream of the VVVVTP outfaU. After the visual inspection, the water quality data was measured with aYSImeter, The water temperature inthe creek upstream of the WWTP outfall was 11,45 degrees Celsius; the dissolved oxygen concentration was 10.25 rn/L.; the specific conductivity was 199 um/crrr; and the pH was 7,57. The water temperature in the creek downstream of the WWTP outfall was 11,88 degrees Celsius, the dissolved oxygen concentration was 10.25 mgliu the specific conductivity was 293 um/cm; and the pH was 7.70. Ron Eubanks contacted Jay Wilson (Charlotte Engineering & property Mara emdnt/Erosion Contrcl Section) concerning the turbidity concerns in the creek. Jay .Wilson indicated that the turbidity was likely being caused by the Charlotte -Mecklenburg Utilities (CMU) sanitary sewer installation work in the area. The DMRs appeared to be properly completed except for the notation of holidays. If the permittee is observing a holiday please enter "Holiday' through all parameters which will have missing results due to the holiday. For example, if temperature is to be monitored daily and a temperature reading will not be taken due to a holiday, enter "Holiday" in the temperature space on the holiday so it can be properly flagged within the Division compliance monitoring database Monitoring violations are being flagged for the Willows Creek WWTP for weeks with holiday because of the lack of "holiday" being noted on the DMR, FACILITY SITE REVIEW: A fence and locked gate surrounds the treatment works for security. The facility's auto -dialer (high water alarm) and other electrical equipment are maintained in a locked building. Some of the facility's components were not in service during the inspection. The plant was not discharging during the inspection. The tertiary filters were backwashing and the ORC left the bypass valve open, which allowed the filter backwash to enter the clear welL The ORC was sending the effluent in the clear well back through the treatment process. FLOW MEASUREMENT: Flow measurement is accomplished using a V-Notch weir as the primary device and a totalizer/continuous recording 1500 4210 Ultrasonic flow meter in accordance with NPDES FLOW MEASUREMENT (cont permit requirements. The flow meter was calibrated on July 9, 2010 LABORATORY: Aqua North Carolina (Heater Utilities, Inc) s certified by the Division of Water Quality to perform field parameter laboratory analyses (NC Laboratory Certification #5035) and contracts with Water Tech Labs, Inc, (NC Laboratory Certification #50) to perform ail other chemical analyses specified tn the permit Water Tech Labs, Inc was not evaluated during the inspection. EFFLUENT/RECEIVING WATERS: The facility was not discharging at the time of the inspection. The facility discharges to an unnamed tributary to Reedy Creek, which is a Class C water in the Yadkin -Pee Dee River Basin. No impacts were observed in the receiving stream. SELF -MONITORING PROGRAM: Discharge Monitoring Reports (DMRs) were reviewed for the period August 2009 through July 2010. The reports indicate that the facility was in compliance with permit effluent limits and monitoring requirements during the review period except for the following: • No dissolved oxygen readings were reported for July 2010. The permittee is required to monitor dissolved oxygen weekly under the permit. These violations were addressed under separate cover by the Division of Water Quality through the issuance of a Notice of Violation (Tracking Number NOV-2010-MV-0154). OPERATIONS & MAINTENANCE: The facility and site appeared well operated, but poorly maintained The vault doors for the valves at the lift station are missing. The walkway grating over the aeration basin has corroded to the point of failure, which is safety hazard The inspector's foot fell through the grating at one point during the inspection. A pump is out in the surge tank and has been down for two rnonths. A UV light bulb is not operating because of a failed ballast and one of the facility's blowers is not operational. The ORC could not recall when the media in the tertiary filters had been replaced. Please repair all malfunctioning components and areas of corroded grating. Replace media in tertiary filters if necessary. SLUDGE DISPOSAL: Sludge is removed from the digester and taken off site by pump and haul methods, Sludge is hauled by Biotech, Inc, or L & L and is disposed of at an approved POTW (CMU) disposal site, BioTech, Inc. and L & L were not evaluated under this inspection SEWER OVERFLOW: Please be advised that pursuant to Part 11, Section E of your NPDES permit and North Carolina Administrative Code (NCAC) 15A 2B 0506 (a)(2), any failure of a collection system, pumping station or treatment facility resulting in a bypass without treatment of all or any portion of the wastewater shall be reported to the appropriate regional office (Mooresville Regional Office SEWER OVERFLOW (cont`d): (704) 663-1699) as soon as possible but no later than 24 hours from the time the peree became aware of the bypass. A Notice of Deficiency was issued on October 16, 2007, PERMIT: The current NPDES permit became effective on December I., 2003, and expires on November 30, 2008. The permit properly describes the facility and appears adequate for the waste stream being treated., The 0.150 MGD permitted facility consists of a package extended aeration wastewater treatment plant with bar screen, flow splitter box, dual surge tanks, aerobic digester, dual aeration basins, dual circular clarifiers, tertiary filters, mud well. UV disinfection system, backup chlorine contact basin with tablet chlorination and dechlorination, and post aeration and effluent continuous recording flow measurement. The certification classification for NC0065773 eq s that the facility have an operator in responsible charge (ORC) with a Grade 2 Biological Water Pollution Control System (WW-2) certification. The facility is staffed with personnel (ORC and Back-up ORC) that possess the appropriate operator certification. RECORDS/REPORTS: Discharge Monitoring Reports (DMRs) were reviewed for the period August 2007 through July 2008. The reports appeared to be properly completed. The ORC maintains detailed logs and calibration records relative to the operation & maintenance and self -monitoring of the facility. FACILITY SITE REVIEW: All of the existing components of the facility were in service and appeared to be operating properly at the time of the inspection. A fence and locked gate surrounds the treatment works for security. The facility's auto -dialer (high water alarm) and other electrical equipment are maintained in a locked building. FLOW MEASUREMENT: Flow measurement is accomplished using a V-Notch weir as the primary device and a totalizer/continuous recording ISC-0 4210 Ultrasonic .flow meter in accordance with. NPDES permit requirements. The flow meter was calibrated on August 27, 2008. LABORATORY: Aqua North Carolina (Heater Utilities, Inc.) is certified by the Division of Water Quality to perform field parameter laboratory analyses (NC Laboratory' Certification #5035) and contracts with Water Tech Labs, Inc. (NC Laboratory Certification 450) to performall other chemical analyses specified in the permit. Water Tech Labs, Inc. was not evaluated during the inspecti EFFLUENT/RECEIVING WATERS: The effluent was clear and did not appear to have a detrimental effect on the receiving stream, at the time of the inspection. The facility discharges to an unnamed tributary to Reedy Creek, which is a Class C water in the Yadkin -Pee Dee River Basin. SELF -MONITORING PROGRAM: Discharge Monitoring Reports (DMRs) were reviewed for the period August 2007 through July 2008. The reports indicate that the facility was in compliance with permit efflt n limits and monitoring requirements during the review period. OPERATIONS & MAINTENANCE: The facility and site appeared well operated and maintained except for the following: The vault doors for the valves at the lift station are extrernely damaged. Please repair the vault doors. SLUDGE DISPOSAL: Sludge is removed from the digester and taken off site by punip and haul methods. Sludge is hauled by Biotech, Inc. or L & L and is disposed of at an approved POTW (CMU) disposal site. BioTech, Inc. and L & L was not evaluated under this inspection. SEWER OVERFLOW: The facility had an unanticipated bypass occur on March 2, 2008. The bypass was caused by a blockage of the bar screen at the WWTP which resulted in the discharge of 500 gallons of untreated wastewater to an unnamed tributary to Reedy Creek, a Class C water in the Yadkin -Pee Dee River .Basin. This violation was addressed under separate cover by the Division of Water Quality through the issuance of a Notice of Violation (Tracking Number NOV-2008-DV-0102). Please be advised that pursuant to Part II, Section E of your NPDES permit and North Carolina Administrative Code (NCAC) 15A 2B .050.6 (a)(2), any failure of a collection system,. pumping station or treatment facility resulting in a bypass without treatment of all or any portion of the wastewater shall be reported to the appropriate regional office (Mooresville Regional Office 704/663-1699) as soon as possible but no later than 24 hours from the time the permittee became aware of -the bypass. A Notice of Deficiency was issued on October 17, 2006, PERMIT: The current NPDES permit became effective on December 1, 2003, and expires on November 30, 2008. The permit properly describes the facility and appears adequate for the waste stream being treated. The 0.150 MGD permitted facility consists of a package extended aeration wastewater treatment plant with bar screen, flow splitter box, dual surge tanks, aerobic digester, dual aeration basins., dual circular clarifiers, tertiary filters, mud well, UV disinfection system, backup chlorine contact basin with tablet chlorination and dechlorination, and post aeration and effluent continuous recording flow measurement. The certification classification for NC0065773 requires that the facility have an operator in responsible charge (ORC) with a Grade 2 Biological Water Pollution Control System (WW2) certification. The facility is not staffed (ORC and back-up) with personnel that possess the appropriate operator certification. Although multiple (13) Back-up ORCs have been designated for the system an ORC has not been designated. Please ensure that a valid ORC is designated for the facility using the attached form. In addition, the following designated Back-up ORCs listed for the Willows Creek Subdivision WWTP are invalid (as noted in the DWQ compliance database) because the operators have not paid their certification renewal fees [David W. Sleigh — January 1, 2007), David Hayes (Invalid — January 1, 2007), and Robert Slusser (Invalid — January 1, 2006). It is the responsibility of System Owners for classified water pollution control systems to designate operators (Operator in Responsible Charge and at least one back-up operator) certified by the Water Pollution Control System Operators Certification Commission of the appropriate type and grade for the system as per 15A NCA.0 8G.0201 (Responsibilities of System Owners) and the standard conditions of NC0065773, RECORDS/REPORTS: Discharge Monitoring Reports (DMRs) were reviewed for the period August 2006 through August 2007. The reports appeared to be properly completed. The ORC maintains detailed logs and calibration records relative to the operation & maintenance and self -monitoring ofthe facility. FACILITY SITE REVIEW: All of the existing components of the facility were in service and appeared to be operating properly at the time of the inspection with the exception of the high water alarm for the mud well; which was not operating when triggered at the time of the inspection. A fence and locked gate surrounds the treatment works for security. The facility's auto -dialer (high water alarm) and other electrical equipment are maintained in a locked building. FLOW MEASUREMENT: Flow measurement is accomplished using a 'V-Notch weir as the primary device and a totalizer/continuous recording NCO 4210 Ultrasonic flow meter in accordance with NPDES permit requirements. The flow meter was calibrated on. July 7, 2007. LABORATORY: Aqua North Carolina is certified by the Division of Water Quality to perform. field parameter laboratory analyses (NC Laboratory Certification #5035) and contracts with Water Tech Labs, Inc. (NC Laboratory Certification #5 0) located in Granite Falls, NC) to .perfonn all other chemical. analyses specified in the permit. Water Tech Labs, Inc. was not evaluated during the inspection.. EFFLUENT/RECEIVING WATERS: The effluent was clear and did not appear to have a detrimental effect on the receiving stream at the time of the inspection. The facility discharges to an unnamed tributary to Reedy Creek, which is a Class C water in the Yadkin -Pee Dee River Basin, SELF -MONITORING PROGRAM: Discharge Monitoring Reports (DMRs) were reviewed for the period August 2006 through August 2007. The reports indicate that the facility was in compliance with permit effluent limits and monitoring requirements during the review period, with the following exceptions: A BOD concentration of 9.5 mg/1 was reported on August 15, 2006, which exceeded the permitted daily maximum limit of 7„.5 mg/1. This violation was addressed under separate cover by the Division of Water Quality through the issuance of a Notice of Violation (Tracking Number NOV-2006-LV-0597). No weekly ammonia -nitrogen concentrations were reported on the August 2007 DMR. Please submit an amended DM.R or provide an explanation for the monitoring violations. According to the ORC, flow proportional composite samples are being collected, as required by the permit. OPERATIONS & MAINTENANCE: The facility and site appeared well operated and maintained except for the following: the high water alarm. for the mud well was not operating when triggered at the tithe of the inspection. Please have the high water alarm repaired as soon as possible. SLUDGE DISPOSAL: Sludge i removed from the digester and taken off site by pump and haul methods.. Sludge is hauled by Biotech, Inc. and is disposed of at an approved POTW (CMU) disposal site. BioTech, Inc. was not evaluated under this inspection. SEWER OVERFLOW: Please be advised that pursuant to Part. II, Section E of your NPDES permit and North Carolina Administrative Code (NCAC) 1 SA 2B .0.506 (a)(2), any failure of a collection system, pumping station or treatment facility resulting in a bypass without treatment of all or any portion of the wastewater shall be reported t© the appropriate regional office (Mooresville Regional Office 704i663-1699) as soon as possible but no later than 24 hours from the time the permittee became aware of the bypass. For spills/bypasses that occur in Mecklenburg County, it is requested that MCWQP be notified at 7041336-5500. Spills occurring on weekends or holidays can be reported by calling the North Carolina Division of Emergency Management at 9191733- 3300, 800/662-79.56, or 800/858-0368. A written report shall also be provided within five (5) days of the time of the incident. The report shall contain a description of the bypass, and its cause; the period of the bypass, including exact dates and times, and if the bypass has not been corrected, the anticipated time it is expected to continue; and steps taken (or planned) to reduce, eliminate, and prevent recurrence of the similar events. Any spill that reaches surface waters (i.e. any spill that reaches any water already present in a conveyance, stream, ditch, etc.) or any spill. greater than 1,000 gallons on the ground that does not reach surface waters must be reported. An adequate spill response for those spills reaching surface waters should include an evaluation downstream of the point. at which the spill entered surface waters to determine if a fish kill occurred. The evaluation should also include the collection of upstream dissolved oxygen and pH measurements for background information and dissolved oxygen and pH measurements at multiplepoints downstream of the entry point to document any negative impact. In your letter under the Permit Section, you reference that your office has not received an annual report for McCarron Subdivision WWTP, I. believe this is a typo 1 assume you meant Willows Creek Subdivision. An annual report was submitted to Barbara Sifford on November 2, 2006 by Leigh Ann Welborn of this office (See Attached). The flow meter was calibrated on July 8, 2006. LABORATORY: Aqua North Carolina is certified by the Division of Water Quality to perform field parameter laboratory analyses (NC Laboratory Certification #503.5) and contracts with Water Tech. Labs, Inc. (NC Laboratory Certification #50) located in Granite Falls, NC. Water Tech Labs, Inc. was not evaluated in this inspection.. EFFLUENT/RECEIVING WATERS: The effluent was clear and did not appear to have a detrimental effect on the receiving stream at the time of the inspection. The facility discharges to an unnamed tributary to Reedy Creek, which is Class C water in the Yadkin -Pee Dee River Basin. SELF -MONITORING PROGRAM: Discharge Monitoring Reports (DMRs) were reviewed for the period July 2005 through June 2006. The reports indicate that the facility was in compliance with perrnit effluent limits and monitoring requirements during the review period, with exception of the following: A BOD concentration of 8..5 mg/1 was reported on May 10, 2006, which exceeded the permitted. daily maximum limit of 7.5 mg/l. This violation was addressed under separate cover by the Division of Water Quality through the issuance of a Notice of Violation (Case Number NOV-2006-LV-0385). According to the ORC, flow proportional composite samples are being collected, as required by the permit. OPERATIONS & MAINTENANCE:. The facility and site appeared well operated and maintained except for the following: the high water alarm for the surge tank was not operating when triggered at the time of the inspection. Please have the high water alarm repaired as soon as possible, Rating: Unsatisfactory SLUDGE DISPOSAL: Sludge is removed from the digester and taken off site by pump and haul methods. Sludge is hauled by Biotech, Inc.. and is disposed of at an approved POTW (CMU) disposal site BioTech, Inc. was not evaluated under this inspection. GENERAL: MCWQP stafflast inspected the facility on February 22., 2005. A Compliance Evaluation Inspection. Report was issued on April H, 2005.. PERMIT: The current NPDES permit becam.e effective on December 1, 2003, and expires on November 30, 2008. The permit properly describes the facility and appears adequate for the waste stream being treated. The 0.150 MGD permitted facility consists of a package extended aeration wastewater treatment plant with bar screen., flow splitter box, dual surge tanks, aerobic digester, dual aeration basins, dual circular clarifiers, tertiary filters, mud well, UV disinfection system, backup chlorine contact basin with tablet chlorination and &chlorination., and post aeration and effluent continuous recording flow measurements. Pursuant to G.S. 143-215..1C, the owner or operator of any wastewater collection or treatment works, the operation of which is primarily to collect or treat municipal or domestic wastewater and for which a permit is issued under this Part, shall provide to the users or customers of the collection system or treatment works and to the Department an annual report. The Mooresville Regional Office of the Division of Water Quality does not possess a copy of the annual report for 2005 for the McCarron Subdivision WWTP. Please provide a copy of the latest annual report. The certification classification for NC0065773 requires that the facility have an operator in responsible charge (ORC) with a Grade 2 Biological Water Pollution Control System (WW-2) certification. The facility is staffed with personnel that possess the appropriate operator certification. However, the Division of Water Quality database indicates that an Operator in Responsible Charge has not been designated (only Back-up ORCs) for the Willows Creek WWTP. Please use the enclosed Water Pollution Control System ORC Designation Form to designate an ORC for the Willows Creek WWTP. .RECORDS/REPORTS: Discharge Monitoring Reports (DMRs) were reviewed for the period July 2005 through June 2006. The reports appeared to be properly completed. The ORC maintains detailed logs and. calibration records relative to the operation & maintenance and self -monitoring of the facility. FACILITY SITE REVIEW: All of the major components of the facility appeared to be operating properly at the time of the inspection, SEWER OVERFLOW: Please be advised that pursuant to Part II, Section E of your NPDES permit and North Carolina Administrative Code (NCAC) 1.5A 2B .0506 (a)(2), any failure of a collection system, pumping station or treatment facility resulting in a bypass without treatment of all or any portion of the wastewater shall be reported to the appropriate regional office (Mooresville Regional Office 704/663-1699) as soon as possible but no later than 24 hours from the time the permittee became aware of the bypass. For spills/bypasses that occur in Mecklenburg County, it is requested that. MCWQP be notified at 704/336-5500. Spills occurring on weekends or holidays can be reported by calling the North Carolina Division of Emergency Management at 919/733- 3300, 800/662-7956, or 800/858-0368. A written report shall also be provided within five (5) days of the time of the incident. The report shall contain a description of the bypass, and its cause; the period of the bypass, including exact dates and times, and if the bypass has not been corrected, the anticipated time it is expected to continue; and steps taken (or planned) to reduce, eliminate, and prevent recurrence of the similar events. Any spill that reaches surface waters (i.e. any spill that reaches any water already present in a conveyance, stream, ditch, etc.) or any spill greater than 1,000 gallons on the ground that does not reach surface waters must be reported. An adequate spill response for those spills reaching surface waters should include an evaluation downstream of the point at which the spill entered surface waters to determine if a fish kill occurred. The evaluation should also include the collection of upstream dissolved oxygen. and pl1 measurements for background information and dissolved oxygen and pH measurements at multiple points downstream of the entry point to document any negative impact.
