Hi Kevin,
Thank you for speaking with me about your facility tests over the phone earlier today.
As discussed, I will enter the data from these past tests into our database.
Please ensure that you carefully review your permit’s Aquatic Toxicity Section [A(2)] for the dual reporting requirements
so that results are both reported on the eDMR and ATForms are sent to the ATForms.ATB@ncdenr.gov email
address. Please also make sure that sampling occurs on the designated months listed in your permit
(Feb/May/Aug/Nov).
Since the testing required for the month of February was not performed, an NOV will be issued and we will require that
a make‐up test is scheduled for early April to avoid potential civil penalties.
After this test, you should then return to your regular quarterly testing schedule and have a test ran for the month of
May. Please make sure that there has been adequate time in between the April and May tests (~2 weeks if possible).

Hi Zach,
I have attached the AT‐2 forms from August 2019 through December 2020. I will mail these forms as well if this is the
only portion of the lab report that DEQ will need.
My understanding is that Cheng submitted a sample from earlier this month that covers our sampling requirement
(which is now on a quarterly basis).

I can confirm that the tests were ran at the frequency indicated in our permit, and we have not seen any of our samples
fail a toxicity test. I will get signed AT‐2 forms sent over to the ATB as soon as I am able. Unfortunately, the ORC for the
site is in a different state and it will not be until early next week that I can get signed forms to you.

I was told by Zach of DWR Aquatic Toxicity Branch (ATB) that toxicity test reports have not been received by ATB since
July 2019. Please check whether toxicity tests were run at required frequency. Thanks.
