Dear Mr. Gentry:
On March 11, 2021, Levi Hiatt and Blane Houck from the Winston-Salem Regional Office of the
Division of Energy, Mineral, and Land Resources (DEMLR), conducted a compliance inspection
for the A&Y Salvage, Inc. facility located at 209 E. Murphy Street, Madison, North Carolina. A
copy of the Compliance Inspection Report is enclosed for your review. The inspection and file
review revealed that the subject facility is currently permitted under a NPDES Industrial
Stormwater Permit, but the facility is no longer operational. The receiving waters for this facility
are designated as the Dan River, a Class V waterbody in the Roanoke River Basin.
As a result of the inspection, the following deficiencies were noted:
1) Failure to Conduct Semi -Annual Qualitative Monitoring
This facility has not adequately conducted semi-annual qualitative monitoring of the
permitted outfalls as required by the NCG100000 permit. Refer to enclosed compliance
inspection report for more information.
2) Failure to Conduct Semi -Annual Analytical Monitoring
This facility has not adequately conducted semi-annual analytical monitoring of the
permitted outfalls as required by the NCG100000 permit. Refer to enclosed compliance
inspection report for more information.
3) Failure to Have a Copy of the Permit and COC Available on Site

This facility did not have a copy of the permit and COC available on site. Refer to enclosed
compliance inspection report for more information.
Required Response
If you wish to close out this permit, address site maintenance and clean-up, then submit a rescission
request. If you wish to keep this permit active, start conducting qualitative and analytical
monitoring as specified by the permit, and keep a copy of the permit and COC available on site. A
response is required within 30 calendar days of receipt of this notice. The response must include
the action that was chosen, and subsequent action plan.
Thank you for your attention to this matter. This office requires that the deficiencies, as noted
above and detailed in the enclosed inspection report, be properly resolved. Failure to address these
deficiencies could result in the issuance of a Notice of Violation, which could be subject to a civil
penalty assessment of up to $25,000 per day for each violation. Should you have any questions or
