The plans must include a general location map, a plan view of the sewer extension, a profile of the sewer extension, and must show the proximity of the sewer extension to other utilities and natural features. Specifications may be omitted for delegated authorities. Each sheet of the plans and the first page of the specifications must be signed and sealed. Three copies of the existing permit if a renewal or modification. c. The appropriate permit processing fee, in accordance with 15A NCAC 2H .0205(c)(5). d. If the owner/authority of the wastewater treatment facility (WWTF) that will be accepting the wastewater flow from this project is different from the applicant of the project, then a letter must be provided from the owner/authority of the WWTF specifying the volume of flow that will be accepted. The letter should be a recent letter and should refer to the project by the same name as that identified on the application and the plans/specifications. e. The form now incorporates the basic information that the Division feels is necessary in order for the delegated municipalities to perform a review on a gravity sewer project for the Division; therefore, the Division will no longer require that the "Delegated Municipalities Gravity Sewer Design Checklist" be completed and submitted. We do, however, require that the delegated municipality perform the same type of review as always required and complete the bottom portion of this page and attach it as the last page of all gravity surer applications that are being submitted as a delegated municipalities project. OFFICE Subject: Application No. WQ0008425 Additional Information Request Fayetteville PWC Country Club Hills, Sect. 4 Sewer Extension Cumberland County Dear Mr. Noland: The Permits and Engineering Unit has completed a preliminary engineering review of the subject application. The following items must be addressed before we can complete our review: 1) In accordance with 15A NCAC 2H .0219 (1) (1), the Division requires that a value of 120 GPD per bedroom be used to calculate the estimated volume of wastewater generated for residential dwelling units (DU). Please indicate the specific number of 3 and 4 bedroom DU, and recalculate the total flow volume estimate. If the figure is different from that originally calculated, please revise page 2 of your application and forward 3 signed and sealed copies of that page to my attention. Note: If the total volume estimate doesn't change, please call me with the information regarding the specific number of bedrooms, or just include the info in a letter to my attention. (There is no need to revise the application in that case.) 2) In accordance with 15A NCAC 2H .0219 (i) (2) (G) (ii), a water line must cross at least 18 inches above a sanitary sewer line. If this minimum distance is not possible, the sewer line must be constructed of DIP. This minimum separation is not maintained in the following regions (according to the profile view in your plans): between MH 4+06 and MH 5+38 , and between MH 5+38 and MH 9+98 (on page 4 of your plans). Please specify that the sanitary sewer line should be DIP in these regions and forward 3 signed and sealed copies of the revised page to my attention. Refer to the subject permit application number when providing the requested information. Please submit three (3) copies of all information to my attention at the address below. Also, please note that failure to provide this additional information on or before October 22, 1993, will subject your application to being returned as incomplete, in accordance with 15A NCAC 2H .0208.
