Division of Water Resources' Central and Regional staff has reviewed the application package
received January 21, 2021. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on April 26,
2021.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A and B, or failure to provide the additional information on or before the above requested date
may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Land Application Site Certification (FORM: LASC 06-16):
1. In accordance with the required items in the Item B of the instruction form, an updated accounting
of every land application site's cumulative pollutant loading rate (CPLR) shall be provided for the
transferred fields and field NC-CL-21-01, which was previously permitted as NC-CL-021-01 under
the permit WQ0000057.
2. Fields NC-CL-52-04 is located on parcels 46735 and 37131 owned by Ron and Amy Parker, while
field NC-CL-52-01 is located on parcel 37130 owned by Edward and Yvonne Parker. As these are
different landowners, they do not meet the exception to waive property line setbacks as determined
in 15A NCAC 02T .1108(f). In order to use the full extent of the land application fields as provided
in the application, two reciprocating setback waivers shall be written, notarized, signed and
registered in accordance with 15A NCAC 02T .1108(e). Otherwise, the appropriate setback shall
be provided on the property lines between separately owned parcels, and an updated LASC shall
be provided with the new acreage along with an updated site map.
The application map for field NC-IR-37-04 has a listed "Onsite House" on parcel 4719410114.000
owned by Jeremy and Jessica Fox, who have not signed an LOA. This site shall be considered an
"Offsite House" with the appropriate setback in 02T .1108. In order to use the full extent of the
land application field as provided in the application, a setback waiver shall be written, notarized,
signed and registered in accordance with 02T .I 108(e). Otherwise, the appropriate setback shall be
provided for the house, and an updated LASC shall be provided with the new acreage along with
an updated site map.
4. The application map for field NC-LT-50-01 has a listed "Onsite House" on parcel 93638 owned by
Amanda and Russell Harrelson, who have not signed an LOA. This site shall be considered an
"Offsite House" with the appropriate setback in 02T .1108. An updated map shall be provided
showing this setback. If the establishment of this setback impacts the full extent of these fields, an
updated LASC shall be provided with the new acreage along with the updated map. If utilized, any
setback waiver shall be written, notarized, signed and registered in accordance with 02T .1108(e).
The application map for field NC-LT-53-02 does not appear to show the onsite house located on
parcel 27567 owned by Howard Goodson Jr. and Donna Goodson, as well as an off -site house on
parcel 25129 owned by Lisa Dennison. Additionally, the house on parcel 56926 owned by Jerry
and Sharon Gold is shown as an "Offsite House", but appears to have an "Onsite" setback. All of
these houses shall be shown in a new map with the proper setback as listed in 02T .1108. If the
establishment of these setbacks impacts the full extent of these fields, an updated LASC shall be
provided with the new acreage along with the updated map. If utilized, any setback waiver shall be
written, notarized, signed and registered in accordance with 02T .1108(e).
6. According to the submitted LASC form and application map, Field NC-ST-03-01 is increasing 10.5
acres due to the addition of land application area in the southwest of the property not previously
permitted. This is a significant modification of this field which would require a soils evaluation in
accordance with the requirements in Item B of the instruction form. Please provide a soil evaluation
for this section of field that meets the requirements listed in Item B.

B. Land Owner Agreement (FORM: LOA 06-16):
Upon review of field NC-LT-53-02, there is a new parcel in the middle of the land application area
(Parcel 102876) owned by Matthew Ryan and April Goodson Rummage. In order to use this area
for the land application of residuals, an LOA form shall be submitted from these owners, and
reciprocating setback waivers shall be provided from both landowners in order to waive the setback
between land application areas and property lines in accordance with 02T .1108(e). If these cannot
be obtained, then the land application area shall remove the areas affected by the new parcel and
associated setbacks, and an updated LASC shall be provided with the new acreage along with an
updated site map.
