The following is an abbreviated application form for those applying for a 401 Water Quality
Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or
bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used
for wetland or stream impacts. Please provide five (5) copies of this application and supporting
information as well as a non - refundable application fee to the Division of Water Quality of $240.00
for impacts to lake bottom of less than 1_acre and $570.00 for impacts to la a ottom of greater or
equal to 1 acre.

Keep Existing Trees and Laurels where possible. Arrows indicate proposed retaining wall.
Average height is 48" high with the top of the wall (12" inches above full pond). Total
length is approx. 130' foot long. Proposed material is 16 "x6" concrete wall block setting on
6" of clean stone for a footing. Backfiil will be comprised of 3/4" clean stone. Rip rap will
rest on the pond side footers to stabilize the footings.
