Please note that this letter was addresse4o Mr. Barry Callicutt who is no longer associated with management of the
church property and any future correspondence should be addressed to Mr. Gary A. Gould, Facilities Coordinator at the
same address.
I have reviewed the findings stated in your) letter with Mr. Gould on site and he has agreed that the items listed will be
addressed. The Plan of Action proposed will be to correct the storm water management system construction as listed in
your letter and provide the necessary documentation before January 1, 2008. This time will allow the Church to obtain
the volunteers and funds necessary to co i plete the work.

Dear Mr. Callicutt:
The Division of Water Quality issued a Coastal Stormwater Management Permit, Number SW8961235
to Glad Tidings Pentecostal Holiness, for a High Density stormwater pond to serve Glad Tidings
Pentecostal on 5/22/1997. This permit expired on 5/22/2007. Section .1003(h) of 15 A NCAC 2H .1000
(the stormwater rules) requires that applications for permit renewals shall be submitted 180 days prior
to the expiration of a permit and must be accompanied by a processing fee, which is currently set at
$420.00. If this is still an active project please complete and submit the enclosed renewal application in
a timely manner. If this project has not been constructed and a permit is no longer needed, please
submit a request to have the permit rescinded. If you have sold the project, or are no longer the
permittee, please provide the name, mailing address and phone number of the person or entity that is
now responsible for this permit. Enclosed is a form for change of ownership, which should be
completed and submitted if the property has changed hands.
Your permit requires that upon completion of construction and prior to operation of the permitted
treatment units a certification of completion be submitted to the Division from an appropriate designer
for the system installed. This is to certify that the permitted facility has been installed in accordance with
the permit, the approved plans, specifications and supporting documentation. Please include a copy of
the certification with your permit renewal request and processing fee. Enclosed is a copy of a sample
certification. Also enclosed is a new Operation and Maintenance agreement that should be completed
and submitted along with your renewal application.
You should be aware that failure to provide the Designer's Certification and the operation of a
stormwater treatment facility without a valid permit, are violations of NC General Statute 143-215.1 and
may result in appropriate enforcement action including the assessment of civil penalties of up to
$10,000 per day.

Dear Mr. Chiles:
The Wilmington Regional Office received previously requested information for the Stormwater
Management Permit Application for Glad Tidings Pentecostal Church on April 2, 1997. A preliminary
review of that information has determined that the application is not complete. The following information
is needed to continue the stormwater review:
1. The dimension information is incomplete. Provide specifics on the driveway widths, parking
stall lengths and widths, and sidewalk widths.
2. Are the roof drains represented by those tiny, faint squares on the detail?
3. The new plan shows a new 18 contour added over the existing pavement to force the roof
drainage into the pond. Will this be accomplished by adding asphalt over the existing parking
lot? Have you added this area into the drainage area for the pond? If not, submit revised
calculations.
4. It is not necessary to list the non -impervious grassed areas in the space for the impervious
area breakdown on the application.
5. Peltier Creek is classified SB#, not SA.'

MOREHEAD CITY, CARTERET COUNTY, NORTH CAROLINA
The purpose of this project is to construct a stormwater retention pond to serve the construction of
a new church sanctuary, classrooms, and parking lot for the Glad Tidings Pentecostal Holiness Church. The
area of proposed disturbance is approximately 4 t acres out of a total site area of 13.57t acres. The retention
pond has been sized and will be constructed to collect the first do inch of run-off from the entire new
construction.
The site is partially located within the city limits of Morehead City, Carteret County, North Carolina
and is being developed by the Glad Tidings Pentecostal Holiness Church, owner of the property. The project
site is located on the south side of N.C.S.R. 1117, Country Club Road, east of the intersection of N. C. 24 and
U.S. Highway 70 East. The current site consists of existing brick church facilities, existing parking areas, and
existing storm drainage system. None of this area is to be disturbed as part of this project. Various brick and
block buildings are located on the original property and on the recently acquired property. The new
sanctuary, classrooms, and parking lot are to be constructed west and south of the existing church facility.
The proposed stormwater retention pond shall be constructed south of the new parking lot.
A Sedimentation and Erosion Control Plan has been prepared, submitted, and approved by N. C.
DEHNR on May 15, 1996, project No. 960418CA.
The proposed project is to be built on a portion of the remaining undeveloped land owned and
occupied by the Glad Tidings Pentecostal Holiness Church. The site is bordered on the north by Country
Club Road (NCSR 1117), South by the Atlantic and East Carolina Railroad and a proposed new roadway,
DOT Project No. 980022831, on the West by Taylor Street and on the East by an existing residential
subdivision.
The topography of the site is basically flat, varying about three (3) feet overall, and currently consists
of grassed lawn, asphalt parking, and heavily wooded areas. The existing storm drainage system serves the
existing building and parking facilities. Existing stormwater from County Club Road will be piped directly to
the existing culvert it now drains toward, and discharge from the retention pond through the grass -lined
swales will be connected to the existing drainage outlet.
The site is bounded on the North by N.C.S.R. 1117, Country Club Road, on the South by proposed
N. C. DOT Project No: 980022831, on the west by Taylor Street, and on the east by others (a residential
district) and will not be affected by the work.
The subsurface profile typically consists of soft organic soils; very soft to firm slightly clayey fine
sandy organic silt to depths of 6.5 to 8.5 feet below existing ground surface. The existing ground water is
estimated to be at a depth of 0.5 feet to 3.0 feet below existing grade (based on soil borings) with possible
seasonal fluctuations.
The stormwater collection system herein and on the attached drawings will provide the following
methods for control of sediments and for removal of pollutants from the developed site runoff.
1. Collection of runoff from the site during development of the retention pond with controlled
discharge.
2. Retention of the runoff resulting from the first one (1) inch of rainfall on the parking and roof
areas into the wet pond. Subsequently this runoff will drain over approximately 3 days period
into the respective 247':t grass -lined swale before draining into a temporary sediment trap,
then draining into another 220't grassed -lined swale before draining into a new 24" o RCP
connecting into the existing drainage system at a newly constructed junction box prior to
discharge towards Peletier Creek.
3. A 36'o CMP standpipe riser is provided in the #1 retention pond. For peak runoff
calculations, see attached calculations this report.
4. The retention time for #1 pond is set at 3.13 days using (3) orifice holes at 7/8" equal to
0.0125 sq. ft.
5. The stormwater runoff in excess of the first one (1) inch of rainfall will discharge from
retention pond #1 through the 36" 0 riser at elevation 16.0' towards the outlet and rip rap at
invert 14.4'. Then into a 20' wide outlet spreader to a 20' x 50' vegative filter draining
through approximately 490' ofgrass-lined swale towards the 24" 0 RCP towards the existing
drainage system. The storm pool elevation is set at 16.0' above mean sea level with
emergency discharge along a 50' long x 20' wide channel prior to the emergency spillway.
6. The entire project site to be served by the new retention pond and grass -lined swales is
shown on the set of drawings titled "Stormwater Management Plan for Glad Tidings
Pentecostal Holiness Church" sheets 1 of 2 and 2 of 2, prepared by Robert M. Chiles, P.E.
7. No wetlands have been delineated on this site.
8. The project site will be developed under zone IC of the existing Town of Morehead City's
Zoning Ordinance.
9. Seeding specifications shall be as specified in the approved Sedimentation and Erosion
Control Plan.
Owner shall maintain the pond during the construction phase of the project and after construction is
completed for all routine and non -routine maintenance tasks. Owner shall be responsible for the perpetual
care of the stormwater retention pond.
All erosion and sediment control practices shall be checked for stability and proper operation
following every runoff producing rainfall and in no case less than once every week during
construction. All repairs shall be made immediately and shall be in accordance with the
approved Sedimentation and Erosion Control plan.
The retention pond shall be inspected during the construction phase of the project following
every runoff producing rainfall and in no case less than once a week. Check the
embankment, spillway, outlet, and swale on the pond for erosion damage, and inspect the
embankment also for piping and settlement. Make all necessary repairs immediately.
Remove all trash and other debris from the riser and pool area. Periodic inspection of the
discharge orifice header shall assure that the system is not clogged. The retention pond shall
be inspected for a build-up of sediment. If and when the sediment layer exceeds 18 inches
thick on the average above the design grade, then this sediment shall be removed. At the
close of the construction phase of the project, the pond shall be cleaned of excess sediment
to the designed grade and begin functioning as a storm water retention pond. All regrading
or repairing of the side slopes, seeding, mowing and any and all repairs shall be performed
prior to close-out of the construction phase.
The grass -lined swale from the pond's overflow structure shall be properly inspected and
maintained against erosion, weeds, and woody vegetation choking the discharge from the
pond. All seeded areas shall be maintained to sustain the required vegetation. Any and all
bare areas shall be replanted as necessary, with all repairs made immediately.

Owner shall be responsible for all routine and non -routine requirements associated with the
maintenance of the stormwater retention pond.
Upon completion of earth disturbing activities following completion of the work, the owner
shall inspect the site following every runoff producing rainfall. A written log shall be
maintained of any needed repairs noted at the inspection, the name of the Contractor
completing the repairs, and the date such repairs are completed.
1. Mowing. The side -slopes, embankment, buffer area, areas within the fence, (if any
installed) areas adjacent to the emergency spillway and the grass -lined swales shall
be mowed at least once a month during the main growing season and at least two
other times during the year to prevent woody growth and to control weeds. Grass
shall be mowed to a "meadow -like" condition and shall be maintained according to
Section 6.11 Permanent Seeding of the approved Sedimentation and Erosion
Control Plan.
2. Inspections. The stormwater retention pond shall be inspected (at a minimum) on
an annual basis to ensure that the structure operates in the manner originally
intended. Additional inspections shall be conducted during wet weather, or times
of extreme weather to determine if the pond is functioning properly. Inspection
priorities shall include (but not limited to) the following: checking the embankment
for any erosion, subsidence, cracking, bare areas in the grass, and tree growth; the
condition of the emergency spillway and adjacent area; the fence (if applicable) and
adjacent planting area buffers; the accumulation of sediment; clogging of the decant
orifice structure and outlet pipe; the adequacyof upstream and downstream channel
erosion protection measures including any modifications which have occurred to the
contributing watershed, and the pond structure including the stability of the side
slopes. Inspections shall be carried out with "As -Built" retention pond plans.
Debris and Litter Removal. Debris and litter removal from the surface of the pond
shall be included as a part of the periodic mowing operation. Particular attention
shall be paid to any floating debris around the riser and trash rack, and the decant
orifice structure shall be checked for possible clogging. Any trash found within the
area of the ponds, ditch, and fence (if applicable) shall be removed and disposed
of property according the City and County regulations.
4. Erosion Control. Corrective measures and repairs shall be done to the pond's side
slopes, embankment, emergency spillway and grass lined swale where settlement,
slumping, and erosion has occurred. Repair as necessary the areas adjacent to the
pond and swale within the fence area. Repair or replace the rip rap protecting the
channel near the outlet as required to stop the erosion. All repairs shall be done in
accordance with the original plans and specifications.
5. Nuisance Control. Owner shall maintain the retention ponds and adjacent areas so
as to control the possible problems of weeds, odors, algae, and insects. Proper
maintenance shall take care of most problems except in times of extreme dry
weather. Owner shall also use biological control against mosquitos and algae
through the use of fish and fat head minnows when possible in lieu of chemical
applications.
1. Structure Repairs and Replacement, Owner shall replace all deteriorated inlet/outlet
works and decant orifice structure and pipe riser works when deterioration from use
and age causes them to fail and function properly. Owner shall be responsible for
all costs incurred with their replacement. Replacement shall be in accordance with
original plans and specifications.
2. Sediment Removal. Owner shall be responsible for the removal of excess
accumulated sediment in the pond when the storage capacity is significantly
reduced from the design capacity due to the properly designed and functioning
stormwater retention pond. Owner shall be responsible for all incurred costs of
removal and proper disposal of the sediment in accordance with all local, state, and
federal regulation s.Sediment removal/clean out cyclefor stormwater retention pond
is recommended on a ten to twenty year cycle. Owner shall every five (5) years
measure the depth of sediment above design grade to determine rate of
accumulation and to establish a time line for sediment removal.
Owner shall be responsible for all annual costs of routine maintenance and for any and all non -
routine maintenance costs as they occur throughout the life of the stormwater retention pond and
adjacent grass lined swales.

1. All erosion and sediment control practices, and stormwater retention pond shall be checked
for stability and operation following every runoff producing rainfall and in no case less than
once every week during construction.
2. Repair or replace sediment control devices when damaged or when failing to properly
contain sediment. Remove the sediment from the swales and sediment traps when it
exceeds 113 of the depth.
3. The retention pond shall be inspected during construction and when and if the sediment
layer within the pond exceeds 18 inches thick on the average, then this sediment shall be
removed. Upon completion of construction stabilization, the pond shall be cleaned of excess
sediment (to the correct invert) and begin function as a stormwater retention pond.
4. Upon completion of earth disturbing activities following completion
of the work, the owner shall inspect the site following every runoff producing rainfall. A
written log shall be maintained of any needed repairs noted at the inspection, the name of
the Contractor completing the repairs, and the date such repairs are completed.
5. The grass -lined spillway, between the riser and the emergency spillway, and all grass -lined
swales, shall be property maintained to assure no erosion occurs and to prevent any woody
vegetation from choking the discharge from the pond. All seeded areas shall be maintained
such as to sustain vegetation. All bare areas shall be replanted as necessary.
6. Periodic inspection of the discharge orifice header shall assure that the system is not
clogged.

Dear Mr. Chiles:
The Wilmington Regional Office received previously requested information for the Stormwater
Management Permit Application for Glad Tidings Pentecostal Holiness Church on February 19, 1997. A
preliminary review of that information has determined that the application is not complete. The following
information is needed to continue the stormwater review:
1. The orifice design is questionable -not the size, but the way it is arranged in the outlet
structure. Will the 4" pipe be closed ended with the smaller holes drilled in it? Please be more
specific on where the holes will be drilled and how the attach the 4" pipe to the structure.
Most orifices are designed so that they slope down at a 45 ° angle outside of the riser below
the permanent pool water level, automatically providing resistance to trash clogging the
orifices. Yours slopes downward from the permanent pool inside the riser, creating a potential
clogging nuisance condition. A trash rack must be designed for the orifice, or the orifice must
be repositioned so that it is outside of the riser and slopes down below the permanent pool,
with the outlet invert at the permanent pool elevation.
2. Please show roof drain locations for the new building and indicate on the plans that they must
be directed so that they are picked up by the pond. Roof drains may not spill out onto the
pavement on the south or east sides of the new building.
3. Add a bermed area around the new catch basin to keep runoff from the new building and
parking from bypassing the pond.
4. Delineate all wetlands on site, or note on the plans that none exist.

5. The pond design for Pond 2 is based on 90% impervious in your calculations, but you report
that both the impervious and the drainage area are 10,200 square feet, therefore 100%
impervious should be used. The surface area for Pond 2 is deficient..
6. Please refer to the SAIDA table used as either 85 % TSS or 90% TSS, not chart 3 and chart
4. If using Chart 3, the 85 % TSS table, you must provide a 30' wide vegetated filter prior
to discharge into the Swale.
7. The following items were previously requested and not provided in this submittal:
a. Dimension the permanent pool, the buildings, parking and sidewalks.
b. Provide a trash rack for the orifice.
C. Provide spot elevations around the building.
d. The forebay weir must be at least permanent pool elevation, but less than design pool
elevation.
e. Label the 6:1 vegetated shelf, its begin and end elevations, and the type of wetland
species to be planted.
8. The pond information must be split out on the application form. Each pond has its own
column.
9. Please show the calculations for the forebay sizing. It must hold 20% of the total volume
above the permanent pool.
Please note that this request for additional information is in response to a preliminary review. The
requested information should be received by this Office prior to April 5, 1997, or the application will be
returned as incomplete. The return of a project will necessitate resubmittal of all required items, including

NARRATIVE
GLAD TIDINGS PENTECOSTAL HOLINESS CHURCH
Rainfall on the existing church sanctuary and classrooms is collected in an existing system
of catch basins and drainpipe.
Rainfall on the western side parking, roadway and buildings will be collected in a diversion
swale around the new retention pond.
The diversion swale will collect stormwater from the southeast corner of the property
through an existing swale and culvert.
Rainfall on the new building and parking area will sheet flow across the parking lot towards
a rip rap flow dissipating structure into the forebay of the retention pond, stored and
decanted over a period of approximately three (3) days.
The south parking expansion is not planned for construction at present, but will be built at
a future date. Stormwater from this parking area will be collected in a catch basin and
flowed into a separate retention pond decanting over a period of approximately three (3)
days.

the elevations of the permanent pool and design pool, the location of and begin/end elevations
for the 6:1 vegetated shelf, and the forebay.

shallower that the main pond body, separated by a low weir wall of earth or timber. The
elevation of the weir wall should be at least at the permanent pool, but no higher than the
design pool.

the pond. It must be located close to the permanent pool to encourage periodic inundation of
