The project will be completed in compliance with NCDENR Division of Water Quality General

The North Carolina Department of Transportation ( NCDOT) has scheduled to remove and
replace Bndge #128 located on SR 1335 in Columbus County Total project length is 225LF

Permit Site 41 is the replacement of Bridge 128 Columbus County which is a Wooden Timber
Bridge 18 6 foot long 25 3 foot wide It was originally built in 1955 and has deteriorated to the
point necessitating repair The proposed Culvert a 401,17 (22 6 x8 1 ) Corrugated Aluminum
Box that will provide for the passage of the 100 year storm as required under the FEMA
approved MOA The installation of the proposed culvert will yield 40LF permanent Stream
impacts with 20 LF of Temporary Stream impacts for dewatering Less than 0 01 acres of

excavation within wetlands must be completed in order to discharge the 36" CMP. This cut will
reduce scour form the discharge from the 36" CMP. The proposed Culvert will increase the
shoulder width, thus increasing safety for the traveling public. Guardrail shall be installed from
Station 12 +00 to Station 14 +25.
RESOURCE STATUS
In general, there will be minor impacts proposed as a result of the replacement of the culvert
along Bridge 4128 in Columbus County. A Nationwide Permit #14 / NCDENR 401 WQC 3886
is required for the project.
Endangered Species: Plants and animals with Federal classification of Endangered (E) or
Threatened (T) are protected under provisions of Section 7 and Section 9 of the Endangered
Species Act of 1973, as amended. As of September 22, 2010, the U.S. Fish and Wildlife Service
lists eight (8) federally protected species for Columbus County.

A biological conclusion of "No Effect" was reached for all species during the May 15, 2012
survey for protected species. Protected species were not identified, nor, was the general habitat
for each species identified.
REGULATORY APPROVALS
Application is hereby made for a Department of the Army Nationwide Permit 14 / NCDENR 401
WQC 3886 as required by the above - described activities.
The National Register was reviewed in addition to conducting a field view of the project for
significant historic features.
The project impacts are minor in nature and should not remove or degrade existing water quality
uses. Pipe size proposed are the minimum necessary to create a safe highway while
accommodating the 25 -year design storm. There is no record within the NCDOT District 3
Office (Columbus County) that the road experiences flood overtopping. By utilizing the erosion
control measures, downstream water quality standards will be protected. All appropriate BMP's
will be utilized during construction.
Attached is a Preconstruction Notification Form, plan sheets, marked county road map, quad
location map and photographs.

NCDOT has an over abundance of Bridges in need of replacement throughout the State. This Bridge has been reviewed
and it has been determined that it's hydrologic function will remain the same if replaced with an accommodating sized
Culvert. This is significantly less expensive than replacement with another Bridge. Thus, allowing NCDOT to replace
more Bridges for the Safety of the traveling public.

Impacts will be minimized as much as possible. The existing Bridge will be removed. The new Culvert will be installed
and all work shall be done utilizing NCDOT best management practices.
