Remove existing dock structure and replace with new concrete style dock
of similar size and in same location. This is to include a walkway, platform
with 4 wide steps on the inside for launching kayaks. A covered boat lift
area that is proposed to have a flat decked area over the boat lift with
safety railing and wooden steps down to a landing area off of the
walkway. Also installation of a single piling for an Osprey nest.
Please give a couple of dates you are available for a meeting.

c. Describe the proposed construction methodology, types of construction equipment to be used during construction, the number of each type
of equipment and where it is to be stored.
Concrete style dock will be constructed with traditional methods and use of hand/power tools along with a barge and
exavator. Pilings will be driven with the use of a vibratory hammer. Equipment will remain on site until project completion.
d. List all development activities you propose.
Replace the existing dock with a new concrete style dock of similar size in the same location. Dock will have a covered boat
lift that will consist of a flat deck with safty railing.

In addition to this completed application form, (MP-1) the following items below, if applicable, must be submitted in order for the application
package to be complete. Items (a) — (0 are always applicable to any major development application. Please consult the application
instruction booklet on how to properly prepare the required items below.
