In accordance with your written request received March 15, 2021, we are forwarding herewith a
corrected Attachment A of Permit No. WQ0035207 issued November 3, 2020 and certified on March 10,
2021.
The correction to the subject permit is as follows:
Inclusion of footnotes 6 through 8 on Attachment A previously included in the attachment
which were erroneously removed in the March 10, 2021 Attachment A Certification issuance.
Please replace Attachment A of Permit No. WQ0035207 issued November 2, 2020 and
certified on March 10, 2021 with the enclosed corrected Attachment A.

ATTACHMENT A — APPROVED RESIDUAL SOURCES Certification Date: March 10, 2021

1. Maximum Dry Tons per Year is the amount of residuals approved for land application from each permitted facility.
2. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition IV.2.
3. Testing of metals and nutrients as stipulated under permit Condition IV.3.
4. Analyses of pathogen reductions as stipulated under permit Condition IVA.
5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.

6. Class A water treatment plant residuals are exempt from meeting the pathogen reduction requirements and the vector attraction reduction requirements in Condition IVA, provided they are not
mixed with residuals that have been generated during the treatment of domestic wastewater, the treatment of animal processing wastewater, or the biological treatment of industrial wastewater. The
Class A water treatment plant residuals must be treated and processed in a manner as not to meet the definition of "Biological Residuals" as defined in 15A NCAC 02T .1102(6).
7. Residuals are generated during the biological treatment of industrial wastewater with no domestic waste streams contribution and meet the definition of "Biological Residuals" as defined in 15A
NCAC 02T. 1102(6). However, it was demonstrated that the residuals are pathogen free and are not required to monitor for Vector Attraction Reduction in accordance with 15A NCAC 02T .1107(c)
provided that no significant odor complaints or problems with vector attraction are encountered and Fecal Coliform Concentrations remain less than 1,000 MPN per dry gram of residuals.
8. Williams WTP residuals are taken to Brown WTP in Durham, where they are mixed, dried, and processed.
If no land application events occur during a required sampling period (e.g., no land application occur during an entire year when annual monitoring is required), then no sampling data is
required during the period of inactivity. The annual report shall include an explanation for missing sampling data. Those required to submit the annual report to United States Environmental
Protection Agency (US EPA) may be required to make up the missed sampling. Contact the US EPA for additional information.
