REQUEST FOR MORE INFORMATION
Dear Sir or Madam:
On March 29, 2006, the Division of Water Quality (DWQ) received your application dated March 29,
2006, to impact 0.47 acres of isolated wetland to develop the site. On Apri16, 2006, the DWQ sent you
correspondence requesting additional information. To date, we have not received the requested
information. Your agent, Sean Hein of Blackburn Engineering, has requested a meeting with the DWQ
and the U.S. Army Corps of Engineers, however, in conversation with personnel with the U.S. Army
Corps of Engineers in their Raleigh Field Office, it seems that some sections of your Pre-Construction
Notification (PCN) application may not be accurate. Specifically, there appears to be some project
history that has not been accounted for in Section V. Prior Project History of your submitted PCN
application. Please carefully review the past project history for this site and resubmit your revised
application on the attached March 2005 version of the PCN application. Additionally, please provide the
information requested below. The DWQ will require this additional information in order to process your
application to impact protected wetlands and/or streams on the subject property. Therefore, unless we
receive five copies of the additional information requested, we will have to move toward denial of your
application as required by 15A NCAC 2H .0506 and will place this project on hold as incomplete until we
receive this additional information. Please provide the information requested above as well as the
following information requested below, so that we may continue to review your project. It is the opinion
of this Office that we will need the requested information prior to scheduling a meeting with you to
discuss the project.
Additional Information Requested:
You have applied for a NW39 (GC3402), but stated that the impacts are to isolated wetlands. We
cannot be certain based upon your site maps whether these wetlands are truly isolated. It appears
that a stream runs near the property. Please clarify whether these wetlands are isolated or 404
jurisdictional, and which permit you are requesting. Additionally, please provide the signed
Jurisdictional Determination for this site.
2. This project will require a stormwater management plan. Per GC3402, since your proposed
imperviousness will be greater than 30 percent and since the site is located in Nutrient Sensitive
Waters (NSW), you must utilize a constructed wetland or bio-retention pond followed by a

forested filter strip and a diffuse flow stormwater BMP device such as a level spreader. Please
provide said plan, or at the very least, a preliminary plan with calculations demonstrating that an
adequately sized facility can be accommodated based upon your site configuration.
Please contact the DWQ within three weeks of the date of this letter to verify that you have received this
letter and that you remain interested in continuing to pursue permitting of your project and will be
providing the DWQ the requested information at a later date. Please contact me in writing and Eric Kulz
of the DWQ Raleigh Regional Office. If we do not hear from you within three weeks, we will assume
that you no longer want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
authorized (at this time) by the DWQ. Please call Ms. Cyndi Karoly or Mr. Ian McMillan at 919-733-
