Good morning Greg,
I received the attached letter, please complete the Rescission form below, unless the permit should be
transferred to Vulcan Material Company then complete the Name/Ownership change form.
Rescind an Existing Industrial Permit
Complete the Rescission Request Form and mail one hard copy of the form to Suzanne McCoy, 1612 MSC,
Raleigh, NC 27699-1612. In addition, e-mail the form to Suzanne McCoy. There is no fee for industrial permit
rescissions. Please note that it can take 1-2 months to process the request because we cannot rescind the
permit until DEQ staff conduct a site visit.
To transfer the permit complete the Name/Ownership Change Form.

owners control.

Subject: NPDES Stormwater Permit Coverage Renewal COC 4NCG160115
Dear Permittee:
For coverage under Stormwyater General Permit for NCG16.0000, the Division of Energy,
Mineral, and Land Resources (DEMLR) is notifying you that your reissued Certificate of
Coverage (COC)-is available. This permit is reissued pursuant to the requirements of North
Carolina General Statute 143�215.1 and the Memorandum of Agreement between the state of
North Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as
subsequently amended.) You must do three things:
(1) You must access and print -a copy of your new Certificate of Coverage (COC) via this
website: https:Hedoes.deg.nc.gov/Forms/SW-COC-Renewal
(2) Also, you must obtain a copy of the new NCG160000 General Permit fiom our website
here: https://deq.nc.gov/about/divisions/enerm,-mineral-land-resoui•ces/npdes-stormwater-aps.
In addition to the full permit, the 2019 print package on the website includes revised monitoring
forms.
The General Permit authorizes discharges of stormwater, and it specifies your obligations with
respect to discharge controls, mpaggPment, monitoring, and record keeping. Please review the
new permit to familiarize yourself with all changes in the reissued permit. Your facility has six
months from receipt of the permit to update your Stormwater Pollution Prevention Plan (SPPP)
to reflect any new permit requirements.
.(3) In preparation for electronic DMR (eDMR) reporting, please enter your stormwater
