To Whom It May Concern,
On behalf of Appalachian State University, we are submitting a Stormwater Management Plan
(SMP) Application for the referenced project. The site is an existing daycare facility that will be
expanding its building footprint and improving parking and traffic circulation. The result of the
expansion required a 401 Water Quality Certification which was granted with conditions (DWR
#20200705) on September 28, 2020. Condition 4 required that the project secure an approved
SMP from NCDEQ-DEMLR.
Please find enclosed the following items for your review.
• Stormwater Management Plan Application Form (Original and Copy)
• Supplemental Form and O&M Agreement for Each BMP (Original)
• Permit Processing Fee ($505, Check No. 50249004)
• Hydrology and Erosion Control Study
• USGS Map
• Engineering Drawings
• Report of Geotechnical Exploration, Wood Environment & Infrastructure Solutions, Inc.
• Deed (Book 2150, Page 777)
Stormwater from the site will be collected, treated for stormwater quantity/quality as required by
Town of Boone and State of North Carolina ordinances, and discharged to an existing stream on
site. If beneficial, a copy of the Town's stormwater approval can be provided when available.
