Application Sets and Public Notice
Dear Mr. White:
On July 20, 2006 the NC Division of Water Quality (DWQ) received one copy of Application for Department of the
Army Permit for the above-mentioned project. An Individua1404 Permit will be required for this project. Please note that
the following must be received prior to issuance of a 401 Water Quality Certification:
1. The 401 Certification cannot be processed until five (5) complete sets of the application and associated maps
are received at the DWQ Central Office in Raleigh. Please send 4 more complete application copies. We
acknowledge receipt of the appropriate fee.
2. The 401 Certification cannot be processed until a copy of the Corps of Engineers Public Notice for this project
is received at the DWQ Central Office in Raleigh. No action is required on your part. The Corps of Engineers
will forward the Public Notice to this Office when it is published.
Until the information requested in this letter is provided, I will request (by copy of this letter) that the Corps of
Engineers place this project on hold. Thank you for your attention. If you have any questions, please contact Ian McMillan
