On the ex emng of July 19 and Jul) 20 2012 a flash flood occulted in the Indian Creek
watershed near Maggie Valley The flash flood resulted in Indian Creek overtopping its banks
and flowing across and down the subject road There was c \tetrstve damage along the road
shoulders dry ditches, as Nell as an adjacent channel (roadside ditch) for an unnamed trtbutat)
to Indian Creek There was less e \tensrve damage 1t several other points along the shoulder and
stteambank tntetface where the water tetutned to Indian Creek fiom SR 1301
On July 20, 2012, the North Carolma Department of Ztanspontatton ( NCDOT) worked to
coordination v%tth Hq)Niood Count) Emergent) Management and tetuuied Indian Creek back
into its original channel to restore access to tesrdents %Nho were cut oft fiom then homes clue to
this catastrophic flood event This letter and PCN application is to folloxN up on out repotting of
the subject events and to make a request to complete additional tepaus to the roadway that affects
jurisdictional areas

The North Catoltna Natural Henrtage Database was checked for records of tlueatened and
endangered species There are no records of threatened or endangered species for the Indian
Creek watershed Additionally there are no records listed for the adjacent watersheds W)cle

Indian Creek (DWQ Class WS III Ti) is shoNNn on the USGS topographic map as a perennial
stream and has sufficient flow to support fish and other aquatic life The unnamed trtbutat) of
Indian Creek is not slioANn as a petenntal stream on the USGS topographic map but does support
aquatic insects and is considered perennial The runnanmeci tt ibutaty is appro \nriately one toot
NNide with a well defined channel and has a cobble, sand and gravel substtate The channel lacks
vegetation Indian Creek flows apptoxnnately 0 25 miles to Jonathan Creek Jonathan Creel.
