Chris Pullinger Please give a brief project description below.* Chris, This is a lot that had a prior 401 cert in 2006 for the previous owner. Authorized 404 fill impacts for this lot at that time were 0.05 acres of 404 wetlands. The prior owner performed the permitted fill (but did not build) after fulfilling mitigation requirements through NCEEP (currently NCDMS). The 401 approval for this project included Lots 66 and 58 (both owned by Ann Warner at the time). Total permitted fill was 0.09 acres in 2006 (0.05 acres for Lot 66 and 0.04 acres for Lot 58). This also received a Nationwide 18 Permit. Now, the owners of Lot 66 are Sarah and Chris Folliard. They would like to build a residential home that has been designed, and in order to do this, they propose to fill and additional 400 sq.ft. (.009 ac) of 404 wetlands as shown on the attached plan. I have been in communication with Billy Standridge, USACE about this current request. This is not a waterfront property and constitutes less than 0.1 ac of wetlands total (past and current permit request combined). I believe this to be notification to DWR only, but am available to meet if necessary. Thanks, Brian Rubino Please give a couple of dates you are available for a meeting. 2/25/2021 2/15/2021 Please attach the documentation you would like to have the meeting about. hines-pamlico point -lot 66-rev prelim-01 -31 - 625.09KB 21.pdf USGS_OCRACOKE.pdf 1.14MB NRCS_Soi I_Ma p. pdf 320.16KB pdf only By digitally signing below, I certify that I have read and understood that per the Federal Clean Water Act Section 401 Certification Rule the following statements: • This form completes the requirement of the Pre -Filing Meeting Request in the Clean Water Act Section 401 Certification Rule. • I understand by signing this form that I cannot submit my application until 30 calendar days after this pre -filing meeting request. • I also understand that DWR is not required to respond or grant the meeting request. TREATMENT STANDARD PROPOSED — TS-1 (TERTIARY TREATMENT WITHOUT NITROGEN REDUCTION) TREATMENT SYSTEM PROPOSED — Puraflo PEAT BIOFILTER SYSTEM INNOVATIVE APPROVAL NO. IWWS-98-1—R3 IMPORTANT NOTICE TO OWNER FROM INNOVATIVE APPROVAL - The Puraflo Peat Biofilter is a pre—engineered proprietary treatment system. The containment modules are made of pre —assembled, UV —protected polyethylene material, with approximate dimensions of 7.08 feet long by 4.58 feet wide by 2.5 feet deep. The modules are fitted with UV —protected, removable polyethylene lids, which contain ventilation and drain holes. Each module in the system is installed at the exact same elevation, with effluent flow uniformly split between the separate modules. The fibrous peat treatment media is residues of Eriophorum (Cotton Grass) plants, extracted from raised bog peats (other natural residues and peat fines are also present). The estimated life of the peat media Is currently 15 years. The media may need to be replaced, in part or in full, In order to maintain specified treatment standards (see note below) TREATMENT PERFORMANCE STANDARDS (TS-1): Tertiary treatment without nitrogen reduction. The Puraflo Peat Biofilter shall be designed, installed and operated to meet the following standards: 1. Biochemical Oxygen Demand, 5—day (BOD5) < 15 mg/L 2. Total Suspended Solids (TSS) < 15 mg/L 3. Ammonium —nitrogen (NH4—N) <10 mg/L 4. Fecal coliform bacteria densities < 10,000 colonies/100 ml (MPN) Standards are arithmetic means, except fecal coliforms is a geometric mean. System classification, management and inspection shall be in accordance with Rule .1961. Puraflo Peat Biofilters shall be classified at a minimum as a Type Va system according to Table V(a) of Rule .1961(b). System Inspections: Both the local health department and the operator —in —Responsible Charge (ORC) must conduct monitoring inspections of Puraflo Peat Biofilter Systems at a minimum frequency as specified in Table V of Rule .1961(b) and the Operation Permit. THIS SYSTEM REQUIRES REGULAR INSPECTIONS BY THE DARE COUNTY HEALTH DEPARTMENT AND THE OPERATOR —IN RESPONSIBLE CHARGE ORC . THE OWNER IS REQUIRED TO CONTRACT WITH A QUALIFIED ORC AND WILL B RESPONSIBLE FOR FEES ASSOCIATED WITH THE CONTRACT AS WELL AS FEES ASSOCIATED WITH ROUTINE WASTEWATER SAMPLING AND TESTING. THE DARE COUNTY HEALTH DEPARTMENT ALSO CHARGES YEARLY INSPECTION FEES. IN ADDITION THE OWNER SHOULD BE AWARE THAT THE TREATMENT MEDIA HAS A LIMITED LIFESPAN AND MUST BE REPLACED AS NEEDED TO MAINTAIN THE SPECIFIED TREATMENT STANDARDS. DISCUSSIONS WITH SEVERAL LOCAL OPERATORS AND INSPECTORS INDICATE THE LIFE SPAN OF THE PEAT IS SUBSTANTIALLY LESS THAN THE 15 YEARS NOTED IN THE INNOVATIVE APPROVAL. THIS APPEARS PARTICULARLY IN SEASONAL RENTAL PROPER77ES WHERE THE ACTUAL FLOW EXCEEDS THE DESIGN FLOW DUE TO OCCUPANCY LEVELS EXCEEDING THE DESIGN STANDARD OF TWO PERSONS PER BEDROOM. 77-IE PROPERTY OWNER IS ADVISED TO S7RIC7LY ENFORCE OCCUPANCY STANDARDS ESPECIALLY IF THIS PROPERTY IS USED AS A VACA77ON RENTAL. / / / / / / / LAKE DRIVE D SITE Cn PROPOSED CONCRETE / DRIVEWAY 6" THICK, 4,000 PSI) / 4 PROPOSED PARKING SPACES \ / 10'x 20' EACH \ \ w 3t,w PUMP,SANK n "E61DGE C6ApED SEPTIC TANK ` BRIDGE LOADED_ 4" SON 80 AT 1%\ PROPOSED / LOT 64 / M FILLED ELEVATIONS\ PAMLICO POINT / P.C.
