Subject: Meeting/email correspondence for emergency replacement of Bridge 136 on SR1561 (Liberty Hill Road)
in Iredell County

A 401 consultation meeting was held on December 11, 2020 between Donna Hood, NC DWR Transportation
Permitting Unit, and Jeff Wyatt, NC DOT Division 12 Environmental Officer to discuss pathways forward and
construction possibilities for 3 bridges that were completely destroyed by Tropical Storm Eta on November 12,
2020.

This on-site consultation included discussion of how to retrieve bridge parts that were washed downstream, how to
reconstruct torn away river/stream banks, and request from DOT to begin debris removal prior to 401 PCN
submittal.

Since this meeting, several phone calls and emails between DWR and DOT have been used for on-going discussion
of permitting and repairing these 3 bridges.

Based on the date of the on-site consultation, the 30 pre-filing requirement has been met.
