Project Description and History 4a.Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:* The landscape in the project area is primarily deciduous forest and agricultural fields.Bridge No.136 was washed out on November 12,2020,during a major flood event.NCDOT proposes to replace the old six span timber and steel bridge with a new 3 @ 90'L x 33'W x 33"D box beam structure on existing location.The project will also require 124 linear feet of streambank stabilization/restoration that was lost during the flood event on the south side of the river. 4b.Have Corps permits or DWR certifications been obtained for this project(including all prior phases)in the past?* r Yes ( No r Unknown 4d.Attach an 8 1/2 X 11 excerpt from the most recent version of the USGS topographic map indicating the location of the project site.(for DWR) Topo Map.pdf 3.12MB 4e.Attach an 8 1/2 X 11 excerpt from the most recent version of the published County NRCS Soil Survey map depicting the project site.(for DWR) Soil Map.pdf 631.13KB 4f.List the total estimated acreage of all existing wetlands on the property: N/A 4g.List the total estimated linear feet of all existing streams on the property: -400 linear feet 4h.Explain the purpose of the proposed project:* Emergency Design-Build Bridge Replacement Project.The purpose of the project is to replace Bridge No.136,which was destroyed by a major flood event on November 12,2020. 4i.Describe the overall project in detail,including indirect impacts and the type of equipment to be used:* NCDOT proposes to replace the bridge with a 3 @ 90'L x 33'W x 33"D box beam structure on existing location.Appropriate BMP's will be installed and maintained according to the approved NCDOT Erosion and Sedimentation Control Plan.Sections of the old bridge structure will be retrieved from the stream channel by accessing the floodplain benches(sand bars)with a trackhoe and/or crane,utilizing crane mats to minimize potential impacts to the active stream channel.Streambank access points will be restored on a 2:1 slope,seeded with appropriate vegetation and stabilized with coir fiber matting.The new bridge structure will have two interior bents,which should be out of the active stream channel upon project completion.The interior bents will require small temporary causeways to construct the 4 drilled piers.Upon completion,the south streambank,which was eroded during the recent flooding,will we restored to pre-existing conditions with soil,vegetation,and Class II rip rap,which will result in the interior bent being on high ground out of the active stream channel. Some minor approach work will be required and the bridge and approaches will be paved.Typical construction equipment includes:trackhoes,bulldozers,dump trucks,cranes,pile driving and drilling equipment,graders,roadway paving equipment,and various hand tools,etc.Attached to the permit drawings is a description of the construction sequence to be implemented in the removal of the old bridge components and trees from the active stream channel. 4j.Please upload project drawings for the proposed project. Impact Justification and Mitigation 1.Avoidance and Minimization la.Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project: The project was designed to minimize impacts to the South Yadkin River.The new bridge will be a 3 @ 90-foot box beam structure on existing location. The two interior bents will be 42-inch drilled piers,which will be located outside the active stream channel once the south stream bank has been restored to pre-flood contours.Appropriate sedimentation and erosion control measures will be installed and maintained as the project progresses according to the approved NCDOT sedimentation and erosion control plan. 1b.Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques: Construction in jurisdictional areas will be limited to the maximum extent practical.Construction of the south interior bent may be constructed using a small temporary causeway in the river to drill the piers;however,once the bent is completed,the south streambank will be restored/reconstructed to pre-flood contours.The streambank will be restored on a 2:1 slope using soil,vegetation,and Class II rip rap,which will surround the south interior bent leaving it on high ground out of the active stream channel.All disturbed ground will be stabilized(seeded,mulched,matted,and/or planted with live stakes)to pre-existing conditions. 2.
