evaluation requirement. Other information in this column indicates vendor and Study identification. An N/A entry
represents parameter exemption from the performance evaluation requirement. A date entered beside the "Fail"
entry represents the completion deadline for this PE follow up.

are not required to submit additional results to this office for the calendar year 2005. Should a PE provider notify
us of a failure on one of your certified parameters, you will be required to analyze and submit a follow up blind PE
result. Failure to analyze and submit an acceptable follow up sample result within 60 days or producing a second
successive unacceptable result may result in decertification for that parameter.
