Dear Mr. Burleson:
On August -6, 2012 the Division of Water Quality (DWQ) received an application set for the above
referenced project. The DWQ has determined that your application was incomplete and /or provided
inaccurate information as discussed below.
Additional Information Requested:
1. DWQ requires five (5) PCN application sets along with the appropriate fee to begin the review
and approval process.
2. Based on your request for written 401 Water Quality Certification approval and the noted stream
impacts of 373 linear feet, the required fee for review and permitting would be $570 made
payable to DWQ.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
