A preconstruction conference was held at the Division Engineer's Office, Edenton, NC, on Thursday, August 9,
2012.

Scott Emory opened the meeting and introductions were made.
Permit Issues:
Bill Biddlecome reviewed the Department of the Army Permit (Permit SAW 2002-1108 1). Bill said that based
on what he has seen so far the Jones pit looks okay. S. T. Wooten plans for this to be used as a commercial pit
for more than just this project. It will provide sand for their asphalt. Bill was not aware of this. S. T. Wooten is
not planning to use this pit to provide borrow for this project at this time. S. T. Wooten plans to clear this pit in
the next 3-4 weeks.
Bill said that the NCDOT needs to make sure the permit plans match the construction plans. Make sure that
permit drawings and conditions are onsite at all times. Bill may perform a compliance inspection at any time.
Bill said to make sure the orange fence is up as required by the DWQ permit. S. T. Wooten is going to use
flagging and stakes to delineate hand clearing areas. Orange fence will be installed at the clearing limits.
Bill explained that there was an in -water work moratorium from Feb. 15'` to June 15'x. S.T. Wooten advised that
due to late start on project that was going to extend the project completion time and that they were going to start
clearing at bridge and culvert sites as soon as possible.
Bill said that it is possible to vary the in water work moratorium for work at the beginning or end of the
moratorium if it will facilitate an earlier completion of the project. This variance will go have to go through the
NC Division of Marine Fisheries.
S. T. Wooten plans to start clearing the project on Monday if riot sooner. Bill requested Clay Willis to send him
a work start email.
S. T. Wooten said their anticipated completion date is now August 5, 2014.
David Wainwright reviewed the 401 Water Quality Certification (Certification No. WQC003875). S. T. Wooten
has shown the silt fence to be offset 3 -5 feet from the toe of slope on their plans in permitted areas to allow for
maintenance. Andy Blankenship said that stakes should be placed at the toe of the slopes in these areas to
delineate the toe of slope.
Paul Williams reviewed DENR Permit Number 63 -12. S. T. Wooten said that they will have to grub the stumps
that are in conflict with the piles at site 10. They will have to walk into that area with equipment and drag out
trees. The logger plans to walk his equipment on the downed trees.
Documentation of the Archaeological recovery at site 31 HF268 is needed.
Turbidity curtains should be used for when driving piles in the water (permanent or temporary) and during the
removal of any piles.
Utilities
The last easement for Dominion Distribution will be obtained tomorrow. Michael Alford said with regards to
utility work that the work in sensitive areas needs to be monitored.

Dominion Distribution plans to have their work completed by the first of November. Roanoke Electric plans to
have their work completed by the end of August.
S. T. Wooten has acquired most of the right of way. The contractor for Centurylink is ready to go as soon as the
contractor has the right of way, has the land cleared and the waterline is installed. Centurylink will meet with
S. T. Wooten next week to determine if they can begin work in certain areas.
Time Warner Cable is starting next week
Michael Alford said that the right of way is staked.
Erosion Control Issues:
S. T. Wooten is using the old erosion control special provisions, therefore, they will have the 21 day seeding
requirements.
Andy Blankenship said to keep the NPDES inspection forms onsitc.

It was requested that the asbestos inspection be pushed through as quickly as possible and that assistance be
given to help with the eviction process.
Other Issues•
Scott Emory reviewed pertinent Special Provisions in the contract. Scott understands that the navigational
clearance only applies to the waterway itself.
Randall Gattis said that AFT will handle the PDA's. Randall said that if the piles on the existing bridge are
really deep they will need to cut them off.
S. T. Wooten did meet the DBE Goals. The first priority for S. T. Wooten at this time is boring the Sewer line at
Mill Branch.
The monthly construction conferences will be held on the 3d Friday of each month at 10 AM at the office trailer.
The next meeting will be September 21".
Scott Emory requested S. T. Wooten to submit a summary of time extensions and additional costs that they are
seeking.
GSC Surveying requested a survey control sheet. Scott said he would forward the request to Casey Whitley with
our Location and Surveys Unit.
